INSTRUCTIONS: This Self-Check Instrument will give the trainer necessary


data or information which is essential in planning training
sessions. Please check the appropriate box of your answer to
the questions below.

1.5.1 Test Devices / systems and/or installation to
determine whether it conforms to requirements
1.5.2 Conduct Stress test to ensure reliability of
equipment in accordance with manufacturer’s
instructions and system requirements
1.5.3 Follow 5S and 3Rs according to environmental
policies
1.5.4 Documentation in relation to the test is
forwarded to appropriate personnel and/or
authority in accordance with requirements
