Dear Dr Williams,
Re: Mr Zach Foster, D.O.B: 25/10/91
I am writing to refer Mr Foster, a 22-year-old builder, who has worsening asthma, for further assessment
including lung function and management.
Initially, Mr Foster presented with the complaints of SOB, wheeze and cough for the last 3 weeks along
with the symptoms consistent with gastro-oesophageal reflux disease. Besides, his compliance with
medications was poor. His physical examination was unremarkable, hence a diagnosis of unstable asthma Commented [ESL1]: ; hence,
associated with GORD was made. Consequently, pulmicort inhaler and pantoprazole was prescribed along Commented [ESL2]: write conditions in full
with an advice to quit smoking and a review visit was scheduled.
On today’s visit, Mr Foster reported improvement of GORD; however he does not take pulmicort regularly Commented [ESL3]: ,
and still smokes. His pathology results including chest x ray and FBE were normal. Therefore, he was
advised to continue his current medications along with several measures for smoking cessation such as
nicotine patch, information brochures and support groups.
Kindly note that Mr Foster is a heavy smoker and is allergic to cats and pollen. In terms of his medical
history, he had been suffering from asthma since the age of 3. His current medications include pulmicort
and Ventolin.
In view of the above, your further assessment and management would be highly appreciated.
