Dear doctor,

Re: Miss Cathy Jones

I am referring Miss Cathy Jones, a 25 years old single lady, whom I suspect having a
ruptured ectopic pregnancy.
Miss Jones presented to my clinic on 15-2-2009 with lower abdominal pain specifically on
the right iliac fossa. There were no fever or acute abdominal signs, but there was quite a
tender right fornix without masses seen during a vaginal examination. She was on POP
contraception when she started her current relationship and has had irregular bleeding.
Therefore she was unsure about her last menstrual period.
However, on review this morning, Miss Jones’s pain worsened overnight and acute
abdominal signs were noted. Cervical excitation and right fornix tenderness found in the
vaginal examination. Her vital signs remained normal. Her pregnancy test returned positive
with clear urinalysis.
I am concerned she has ruptured ectopic pregnancy and would appreciate your urgent
assessment. Please keep me informed of the outcome. Thank you.
