2009 -2010 Federal Direct PLUS Loan Request Form

This form must be completed each academic year to request a PLUS loan. The information that you supply on
this form will be electronically transmitted to the U.S. Department of Education, its servicers and/or agents. The
parent borrower’s credit history will be reviewed to determine eligibility.

Step 1: A parent who wishes to apply for a Direct PLUS Loan for the 2009-2010 academic year must complete all
sections of this form.

Please provide the information of one parent borrower. In order to apply for a PLUS loan, the borrower must
be a natural parent, adoptive parent or stepparent (if the stepparent’s information was reported on the
student’s FAFSA), of a dependent student. Please provide all of the following information; this form cannot be

Please note: Federal regulations require proof of citizenship, if the parent borrower is noted on the student’s FAFSA, we
will use that data to verify citizenship. If parent borrower not listed on the FAFSA than parent will be required to submit
some form of citizenship, i.e. copy of a passport or birth certificate and must complete a PLUS Non Custodial addendum.

 A parent may borrow up to the total cost of attendance minus all the financial aid and resources
received by the student. If the loan is for one semester only, the loan amount is based on the cost of
attendance for that semester minus the financial aid and resources for that semester. If the loan is
for the summer session(s), the loan amount is based on the cost of attendance for the summer 1

session(s) minus the financial aid and resources for the summer.
 All Plus loans will be processed for the full academic year, unless the borrower indicates to us in
writing otherwise, which means the loan amount less fees will be received in two disbursements, one
in each semester.
 Upon receipt of credit approval, parent will receive an email letting them know that they have been
approved for the PLUS and will be advised then to complete their Master Promissory Note.

In the case of a PLUS Denial


In the event that the PLUS loan is denied based on the credit review, please check ONE option below:

____ Allow the student the option to receive the additional Federal Direct Unsubsidized Loan up to their grade
level maximum.
If eligible, I accept the additional Federal Direct Unsubsidized Loan.

Step 2: Complete PLUS Loan MPN


 Complete the Direct PLUS Loan Master Promissory Note (MPN) at www.dlenote.ed.gov upon notification of
credit approval.

It is important to remember the following:


 This is a request for the Parent Direct PLUS loan: it is not a promissory note. A master promissory Note
(MPN) must be completed before the Direct PLUS loan can be credited to your student’s account.
 The Free Application for Federal Student Aid (FAFSA) must be submitted each academic year before a PLUS
loan request can be processed.
 The maximum loan amount a parent may borrow cannot exceed the amount indicated on the student’s
award offer from the University.
 There is a 4% origination fee that is deducted from the gross loan amount and an up front rebate of
1.5%will automatically be applied. To keep this up front rebate, you must make your first 12 payments on
time once you go into repayment.
 The interest rate on a Direct PLUS Loan first disbursed on or after July 1, 2006 will be fixed at 7.9%.
 By signing and submitting this PLUS loan request, you consent to borrow funds under the Direct PLUS loan
program and understand that if approved and after a Master Promissory Note is completed, your funds will
be applied to your student’s account in accordance with the university’s disbursement schedule.
 You may cancel all or a portion of your loan after funds have been credited to your student’s account by
notifying us in writing within 30 days after the date of your loan disbursement notice.

I consent to the U.S. Department of Education and its agents to review my credit report and use the information
from that report in determining whether to award a Federal Direct PLUS loan to me. I understand that I will be
notified of the results of the credit check. I also attest that I have read and agree to all the information on this
form, including the Privacy Act Disclosure Notice contained at the end of this form and certify that I am not in
default of a Federal Student Loan.

Consent to Obtain Credit Report
Privacy Act Disclosure Notice
The Privacy Act of 1974 (5 U.S.C. 552a) requires that the following notice be provided to you. The authority for
collecting the information requested on this form is §451 et seq. of the Higher Education Act of 1965, as amended.
Your disclosure of this information is voluntary. However, if you do not provide this information, you cannot be
considered for a Federal Direct PLUS loan. The information on this form will be used to determine your eligibility
for a Federal Direct PLUS loan. The information in your file may be disclosed to third parties as authorized under
routine uses in the Privacy Act notices called “Title IV Program Files” (originally published on April 12, 1994, Federal
Register, Vol. 59 p. 17351) and “National Student Loan Data System” (originally published on December 20, 1994,
Federal Register, Vol. 59 p. 65532). Thus, this information may disclosed to federal and state agencies, private
parties such as relatives, present and former employers and creditors, and contractors of the Department
of Education for purposes of administration of the student financial assistance program, for enforcement purposes,
for litigation where such disclosure is compatible with the purposes for which the records were collected, for use by
federal, state, local, or foreign agencies in connection with employment matters or the issuance of a license, grant,
or other benefit, for use in any employee grievance or discipline proceeding in which the Federal Government is a
party, for use in connection with audits or other investigations, for research purposes, for purposes of determining
whether particular records are required to be disclosed under the Freedom of Information Act, and to a Member of
Congress in response to an inquiry from the congressional office made at your written request.
Because we request your social security number (SSN), we must inform you that we collect your SSN on a voluntary
basis, but section 484(a)(4) of the HEA (20 U.S.C. 1091 (a)(4)) provides that, in order to receive any grant, loan, or
work assistance under Title IV of the HEA, a student must provide his or her SSN. Your SSN is used to verify your
identity, and as an account number (identifier) throughout the life of your loan(s) so that data may be recorded
accurately.
