Insertion of the airway
Oropharyngeal airways are inserted into the
mouth of adult patients upside down.
Once contact is gently made with the back of the throat,
the airway is rotated 180 degrees.
In children (airways sized 000-1), the airway is inserted into
the mouth right side up to avoid causing trauma to soft
tissues.
The length of the Guedel airway The Guedel airway is best inserted Insert with the curve facing the
can be estimated by the distance when the patient is in the supine opposing direction and follow the
from the patients nostrils to the position. Prior to insertion the airway whilst turning the Guedel

water based lubricant.
Clinical Applications
of Oropharyngeal Airways
Maintain a patent airway
For use in an unconscious patient1
Lifts tongue and epiglottis away from the back
of the throat
Prevents obstruction of the air passage
Prevents patient from biting on tongue, ET Tube,
clinician fingers
Facilitates oral suctioning
Improves mask fit during manual ventilation
1
For use in unconscious patients only.
Oropharyngeal airways are usually indicated for
unconscious patients as there is a high probability that
the device may stimulate the gag reflex in a conscious or
semi-conscious patient.
This could cause the patient to vomit and potentially
lead to an obstructed airway. Nasopharyngeal airways are
mostly used when the patient has a gag reflex, due to the
fact that it can be used on a conscious patient, whereas
the oropharyngeal cannot.

Clinical Guidelines
Size Selection
The correct size of the oropharyngeal airway is
chosen by measuring and aligning the flange of
the airway with the centre of the patients lips and
the tip touching the angle of the jaw.
Should the airway not reach this distance a
larger one should be selected.
Equally, should the airway greatly exceed this
distance, a smaller one should be utilised.

Patient airway is not being
maintained either by the

or the patient has become
