1. You may have a case of food poisoning, I suggest you will stay on a clear
liquid diet, drink plenty of fluids to prevent dehydration. Stay away from
spicy and greasy food to let your stomach settles.
2. I also advise for you not to eat any red meat and any heavy food, such as
greasy and fried food, and stay away from starchy vegetables.
3. My nurse will clean and disinfect the area so I can take a look at the
wound.
4. You are very lucky because the knife did not cut into any major arteries.
You will need some stitches because the wound is quite deep.
5. I feel quite dizzy, is it because I lost too much blood?
6. Its good that your coworkers brought you here immediately, otherwise you
could develop a condition call gangrene. (chng hoi th)
7. Gangrene is when the tissues in your body died and need to be removed,
otherwise it can get infected and could be life threatening.
8. Are you up to date with your vaccinations? When was the last time you had
your tetanus shot?
9. Besides vitamins, are you taking any other herbal supplements?
10. I also take fish oil capsules, not every day only when I remember.
11. My nurse will inject you with a local anesthetic to suture the cut ( khu
vt ct.) Suture (s khu vt thng) are special surgical stitches(khu phu
thut c bit) we use to hold the tissues together.
12. You should change the bandage daily, apply the antibiotic ointment I will
prescribe to you, I will give you some painkiller to bring home in case you
are having some pain.
13. Please look out for any signs of infections, like swelling, drainage,
tenderness and redness.
14. You need to make a follow-up appointment to have the stitches removed.
15. The painkiller will be very helpful once the effect of the anesthesia wears
off.
16. Any surgeries or medical procedures done in the past ten years?
17. Two years ago, I had my gallbladder removed, about ten years ago; I also
had my hernia surgery.
18. I would like to ask you a few questions about your medical history? Do you
have any problems with your kidney, liver or stomach?
19. A few months ago, I was worried I may have kidney stones. I went to see a
specialist in stomach, he did not find anything wrong.
20. Do you have any sores(l lot) on your feet that a long time to heal?
21. Yes I do and they are swollen as well, sometimes I cannot put on my socks.
22. These could be sign of having of diabetes, do you or anyone in the family
have this condition?
23. I dont have it, but my aunt suffered from this disease, the doctor had to
amputate one of her legs.
24. Any other chronic illness or heart disease?
25. My blood pressure is high sometimes, but thats about it.
26. Any problems with your bladder or urinary tract?
27. Recently I noticed occasionally when I urinate; I do feel a burning
sensation. I also see some blood in the urine.
28. I will order some blood work and a urine test, you need to go to the lab
tomorrow.
29. If you dont treat the disease, it can be serious. You can control it by insulin
injections, taking diabetes pills, follow a nutritious diet and exercise
regularly.
30. Can you describe your symptoms? Is the pain constant or does it come and
go? Is it a sharp pain or dull?
31. Since last night, I have been vomiting and having cramps. I am afraid I may
have eaten something bad.
32. How many bowel movements have you had, and how many times have you
vomited?
33. I have had diarrhea since last night, my stools are watery and I have
vomited at least three times. My stomach cannot tolerate anything, not
even water.
34. Is the stomach pain constant or does it come and go?
35. Have you tried to take any medications for this? You can try to take some
anti acid medication; it could neutralize the acid in the stomach.
36. Are your stools watery or soft? Is the vomiting constant or only happens
after you eat something?
37. Are you allergic to any food or medications?
38. Any surgeries or medical procedures done that require you to stay at the
hospital?
39. I had my hernia surgery about 5 years ago, that was the only time I was
admitted into the hospital.
40. Could you provide me with an excuse note since I didnt go to work today?
41. How long have you had the chest pain? Is it a sharp or dull pain? Does it
radiate to anywhere else ?
42. I do feel lots of pressure on my chest, its a sharp pain and it does spread to
my back sometimes. I am concerned if its a sign of having a heart attack.
43. Is this the first time you experience this pain? Have you taken anything for
the discomfort?
44. No I havent, my husband told me to take aspirin, but I am not sure if it
would be safe.
45. Have you ever been diagnosed with heart disease? Like circulatory
disorder or irregular heartbeat?
46. We will send you to the lab to get a Cat scan done for your heart, have you
done any heavy lifting lately?
47. I was in a car accident about a month ago, I hit my chest against the
steering wheel.
48. You may have injured your ribs, we will send you to get an X-ray done as
well.
49. Do you have asthma, tuberculosis, pneumonia ? Do you smoke or drink?
50. I will listen to your heart and lungs, take your temperature and blood
pressure, then the doctor will come in to examine you.
51. You may have pulled your muscle from lifting heavy things, but to be safe,
we should send you to get an X-ray done.
