Instructions: Please rate the child on each of the items on pages 2 and 3 of this rating form.

Ratings should be based on your observation of this childs behavior during the past three months. The rating points after each item appear in the following format: Never Rarely Sometimes Often 0 1 2 3 Never If the child does not exhibit a specified behavior, or if you have not had an opportunity to observe it, circle 0, which indicates Never. If the child exhibits a specified behavior or characteristics, but only very infrequently, circle 1, which indicates Rarely.
