Peptic ulcer is the most common GIT disorder with a prevalence of 2%, peaking around the age of 70 years. Laparoscopic repair of perforated peptic ulcer began to evolve and replace the ordinary upper laparotomy. We studied 47 patients suffering from perforated duodenal or gastric ulcer in Al-Mawanee General Hospital in Basrah in the period 2014-2017, the patients were categorized into 2 groups: 1st group(24 patients) managed laparoscopically and the 2nd group (23 patients) managed by open approach. Perforation found to occur more with duodenal ulcer (29 pt. 61.70%) and more in male patients (26 pt. 55.33%). The peak perforation seen in age group >60 years and the commonest risk factors was the NSAIDs usage. Operative time was insignificantly longer in laparoscopic approach. However, the laparoscopic approach has less post operative pain and less overall complications (4 pt. 16.6% vs. 8 pt. 34.7%). The surface of the human body is nearly devoid of surface linearity. Despite this, most surgical incisions are intentionally straight by design, resulting in conspicuous scar lines. W-plasty and Z-plasty techniques are used after-the-fact to revise aesthetically unpleasing scars, but both are geometric and comprised of linear segments. In nature, random patterns that blend into organic backgrounds become less conspicuous, thus providing the rationale to “redesign” initial surgical incisions. Proactively created truly non-geometric “irregularly irregular incisions” can provide a striking decrease in scar detectability and healed incisions more effectively evade perception. Scar “lines” in such cases become random as a result of the complete jumbling of myofibroblast contraction. As a result, later needs for scar revision are reduced.

Background: Septoplasty is commonly performed to offer qualitative and quantitative advantage to those with nasal obstruction owing to septal deviation. Therefore, the aim of the present study was to assess the post-surgical outcomes of patients undertaken septoplasty with regard to initial clinical complains. Methodology: This study included a series of patients presented with nasal obstruction and subsequently undergone septoplasty. In the present study, patients presented with different clinical complains; 83.2% presented with nasal congestion, 94% with nasal blockage, 87% with breathing trouble, 84% with sleeping trouble, 71% with exercise problem, and 3.8% with other complications (e.g bleeding, loss of smell). Conclusion: In patients with nasal obstruction due to DNS or other causes, nasal septoplasty results in significant improvement in reduction or completely eliminates the prior complications.

Background: Surgical mortality data are collected routinely in high-income countries, yet virtually no low-or middle-income countries have outcome surveillance in place. The aim was prospectively to collect worldwide mortality data following emergency abdominal surgery, comparing findings across countries with a low, middle or high Human Development Index (HDI). Methods: This was a prospective, multicentre, cohort study. Self-selected hospitals performing emergency surgery submitted prespecified data for consecutive patients from at least one 2-week interval during July to December 2014. Postoperative mortality was analysed by hierarchical multivariable logistic regression. Results: Data were obtained for 10 745 patients from 357 centres in 58 countries; 6538 were from high-, 2889 from middle-and 1318 from low-HDI settings. The overall mortality rate was 1⋅6 per cent at 24 h (high 1⋅1 per cent, middle 1⋅9 per cent, low 3⋅4 per cent; P < 0⋅001), increasing to 5⋅4 per cent by 30 days (high 4⋅5 per cent, middle 6⋅0 per cent, low 8⋅6 per cent; P < 0⋅001). Of the 578 patients who died, 404 (69⋅9 per cent) did so between 24 h and 30 days following surgery (high 74⋅2 per cent, middle 68⋅8 per cent, low 60⋅5 per cent). After adjustment, 30-day mortality remained higher in middle-income (odds ratio (OR) 2⋅78, 95 per cent c.i. 1⋅84 to 4⋅20) and low-income (OR 2⋅97, 1⋅84 to 4⋅81) countries. Surgical safety checklist use was less frequent in low-and middle-income countries, but when used was associated with reduced mortality at 30 days. Conclusion: Mortality is three times higher in low-compared with high-HDI countries even when adjusted for prognostic factors. Patient safety factors may have an important role. Aim Previous studies reported conflicting evidence on the effects of obesity on outcomes after gastrointestinal surgery. The aims of this study were to explore the relationship of obesity with major postoperative complications in an international cohort and to present a meta-analysis of all available prospective data. Methods This prospective, multicentre study included adults undergoing both elective and emergency gastrointestinal resection, reversal of stoma or formation of stoma. The primary end-point was 30-day major complications (Clavien-Dindo Grades III-V). A systematic search was undertaken for studies assessing the relationship between obesity and major complications after gastrointestinal surgery. Individual patient meta-analysis was used to analyse pooled results. Results This study included 2519 patients across 127 centres , of whom 560 (22.2%) were obese. Unadjusted major complication rates were lower in obese vs normal weight patients (13.0% vs 16.2%, respectively), but this did not reach statistical significance (P = 0.863) on multivariate analysis for patients having surgery for either malignant or benign conditions. Individual patient meta-analysis demonstrated that obese patients undergoing surgery for malignancy were at increased risk of major complications (OR 2.10, 95% CI 1.49-2.96, P < 0.001), whereas obese patients undergoing surgery for benign indications were at decreased risk (OR 0.59, 95% CI 0.46-0.75, P < 0.001) compared to normal weight patients. Conclusions In our international data, obesity was not found to be associated with major complications following gastrointestinal surgery. Meta-analysis of available prospective data made a novel finding of obesity being associated with different outcomes depending on whether patients were undergoing surgery for benign or malignant disease.

Aims
Emergency surgery is associated with poorer out- comes and higher mortality with recent studies suggesting the 30-day mortality to be 14–15 %. The aim of this study was to analyse the 30-day mortality, age-related 30-day mortality and 1-year mortality following emergency lapa- rotomy. We hope this will encourage prospective data collection, improvement of care and initiate strategies to establish best practice in this area.
Methods
This was a retrospective study of patients who underwent emergency laparotomy from June 2010 to May 2012. The primary end point of the study was 30-day mortality, age-related 30-day mortality and 1-year all- cause mortality.
Results
477 laparotomies were performed in 446 patients. 57 % were aged \70 and 43 % aged [70 years. 30-day mortality was 12, 4 % in those aged\70 years and 22 % in those [70 years (p \ 0.001). 1-year mortality was 25, 15 % in those aged under 70 years and 38 % in those aged [70 years (p \ 0.001).
Conclusions
Emergency laparotomy carries a high rate of mortality, especially in those over the age of 70 years, and more needs to be done to improve outcomes, particularly in this group. This could involve increasing acute surgical care manpower, early recognition of patients requiring emergency surgery, development of clear management protocols for such patients or perhaps even considering centralisation of emergency surgical services to specialist centres with multidisciplinary teams involving emergency surgeons and care of the elderly physicians in hospital and related community outreach services for post-discharge care.
