As expected, since we recently celebrated the 250th anniversary of birth of Wolfgang Amadeus Mozart, there has been again a renewal of interest in his short but intensive life, as well as in the true reason of his untimely dead. Mozart lived and died in time when the medical knowledge was based mostly on subjective observations, without the established basics of standardized medical terminology and methodology. This leaves a great space for hypothesizing about his health problems, as well as about the cause of his death. The medical academic community attributed to Mozart approximately 150 different medical diagnoses. There is much speculation on the possible causes of Mozart's death: uremia, infection, rheumatic fever, trichinellosis, etc. Recently some authors have raised the question about a possible concomitant neurological disease. Initial presentation. A 72-year-old man was admitted to the hospital with a 5-day history of shortness of breath. He initially presented to the emergency department and was found to have significant difficulty with respiration. On the day after admission, the patient developed stridor, tachypnea, and hypoxia, requiring bilevel positive airway pressure (BiPAP) use. ENT was consulted for evaluation of new-onset stridor; direct fiberoptic laryngoscopy revealed proper vocal cord function while awake but bilateral vocal cord abduction paralysis while asleep. Collateral history revealed that the patient had experienced progressive dysphagia, ataxia, and bradykinesia for the previous 3 years. He also experienced constipation, erectile dysfunction, and orthostatic hypotension. His medical history was remarkable for hypertension, diabetes, heart failure, and left frontal and cerebellar strokes.

The first 100 words appear below: A 60-year-old male presented with weakness of the left side of the body followed by a sudden fall on the ground while working in the field, with difficulty in speech for 03 months. Weakness was sudden in onset which involved left upper and lower limbs with the inability to move the left side of the body and difficulty in walking. The patient also noticed that there was a deviation of the face towards the right side associated with dribbling of food with difficulty in speech but can follow the command with difficulty. There was also urinary incontinence for the same duration, but the bowel movement was normal.
Background: The present work aims to characterize the profile of patients with stroke treat at a hospital located in the Region of the Mata of Minas Gerais, Brazil, considering the findings of the clinical vocal tract, kind of stroke, age and gender of such patients. Methodology: To obtain data, the clinical profile of 133 patients with a clinical or tomography diagnosis of stroke was analyzed, and the results were presented in percentage. For quantitative data average and analysis the tests were done with associations that held χ 2 test, and for significance it was considered p < 0.05. Results: From the total of patients, 63 were women, accounting for 47.4% and the other 52.6% were males. Clinically, they were characterized with the highest percentage for ischemic stroke (89.4%) compared to the hemorrhagic type (10.6%). Most of them were referred for computed tomography (86.5%) and remained hospitalized for an average of 6.496 ± 7.372 days. Similar percentages were obtained in the analysis of the population in question, when considering if they had (54.1%) or not (49.6%) any damage in their speech, language skills or swallowing. There were different types of disabilities in patients with stroke. Men with an average age of 69.8 ± 13.9 presents mostly ischemic stroke, and the majority of patients with stroke had hemiplegia and abnormalities of the vocal tract, dysphasia, and aphasia. While older patients had an ischemic stroke and were presented with left hemiplegia, the younger ones suffered from hemorrhagic strokes that caused a disability characterized as right hemiplegic. The changing of musical capabilities, after cerebrovascular or neurodegenerative lesions in neuropsychological literature, are referred, over all, to musicians affected by aphasia. Generally, authors examine the relationship between amusia and aphasia and the potential decays of musical general skills. Particularly uncommon is the investigation of pre-lesional versus post-lesional productions of music composers. The rare descriptions that we find in literature, tend to conclude that the postlesional productions maintain a good creativity level. In the following paper a group of researchersneurologists, musicians and specialists in music composition-are comparing pre-lesional masterpieces of one composer manifesting cerebro-degenerative disease (Maurice Ravel) and four composers suffering cerebrovascular lesion (Benjamin Britten, Alfred Schnittke, Vissarion Shebalin, Franco Donatoni) with their postlesional compositional productions, applying an objective methodology. Their conclusions are that the examined composers employ, in their post-lesional masterpieces, simplified structures, or simple classic models, or reminiscences, influences and citations referred to their previous experience or related to harmonic traditional forms, in a less elaborate structure, frequently fragmented, static and repetitive. The compositional contextual character doesn't change and maintains, in some way, a "poetry", but the application modality becomes more "technical". The Authors suggest that, in order to be objective in her analysis and conclusions, neuro-aesthetic requires the application of well-defined methodologies and scientific procedures.

A 19-year-old man was admitted with a 2-week history of continuous cough along with a day history of acute onset unsteadiness and hiccups. Given the current pandemic, he was initially suspected to have COVID-19, however he tested negative on two occasions. Subsequent brain magnetic resonance imaging (MRI)confirmed a small left acute and subacute lateral medullary infarction with chest X-ray suggesting aspiration pneumonia with right lower lobe collapse. This is a distinctive case of posterior circulation stroke presenting with a new continuous cough in this era of COVID-19 pandemic. We anticipate based on MRI findings that his persistent cough was likely due to silent aspiration from dysphagia because of the subacute medullary infarction. It is therefore imperative that healthcare workers evaluate people who present with new continuous cough thoroughly to exclude any other sinister pathology. A bladed gunstock club sold in 2021 at an Auction in the Normandie without documented provenance is identified as having been collected in 1823 near Fort Lookout by Duke Paul Wilhelm of Württemberg. Another one, also without documented provenance, now in the Warnock Collection is identified as formerly having been in the Westerm Museum in Cincinnati, based on a drawing attributed to the Duke of Württemberg.
