OBJECTIVES: Frequently, the term "quality of life" has been used to justify personal and professi... more OBJECTIVES: Frequently, the term "quality of life" has been used to justify personal and professional decisions in all fields of medicine. Nowadays, quality of life studies are based on development and validation of sensitive measures of patient outcomes, incorporating functional status and perceived health status. Thus, quality of life has become an outcome as important as survival and effectiveness. Arch. Esp. Urol. 2009; 62 (7): 519-530 METHODS: A systematic review using Pubmed and Medline was performed, searching for papers concerning health related quality of life and urology. The most relevant articles where questionnaires and interviews were described and validated were listed. RESULTS: Based on psychometric proprieties, a search between 1970 and 2007 identified a total of 25 recommendable articles with generic inventories and specific modules that have been developed, validated and used in clinical practice or research. Historical aspects, quality of life concepts, validation of questionnaires and structured interviews, and most used instruments in generic health-related quality of life, general urology and urological oncology have been discussed.

Purpose: Some urologists who perform vasectomy reversals are not experienced with performing VE. ... more Purpose: Some urologists who perform vasectomy reversals are not experienced with performing VE. A model to preoperatively identify patients who may require referral to an experienced VE surgeon was created (www.uroengineering.com). We tested the model at multiple institutions. Materials and Methods: The model had previously been designed in 483 patients who underwent vasectomy reversal at 1 institution (100% sensitive and 59% specific for predicting the need for VE). It was based on time since vasectomy and patient age. We tested it prospectively in 33 patients and retrospectively in a total of 312 at 6 other institutions. The predictive accuracy of the model was compared to using a simple duration from vasectomy cutoff alone, as is used in clinical practice. Results: The model had 84% sensitivity and 58% specificity for detecting the need for VE in a total of 345 patients at 7 institutions. If using only a duration from vasectomy cutoff of 10 years to predict the need for VE, sensitivity was only 69%. At a cutoff of 4 years sensitivity was 99% but specificity was only 23%. Thus, the model performed better than any specific duration cutoff alone.

However, the ideal surgical procedure has not yet been developed. Because plaque incision and grafting does not cause penile shortening, it is the best approach, particularly in cases of severe and complex penile curvature. Additionally, if the graft is nonsynthetic, neither inflammatory nor anomalous scarring will develop over time. This review suggests crural corpora cavernosa as the graft of choice, because it more closely resembles the incised area both anatomically and functionally.

We determined the efficacy of atropine sulfate combined with papaverine hydrochloride, prostaglan... more We determined the efficacy of atropine sulfate combined with papaverine hydrochloride, prostaglandin E1 and phentolamine mesylate in the pharmacological erection test. A total of 230 consecutive patients with erectile dysfunction was randomized for pharmacological erection test and received an intracorporeal combination of 50 mg. papaverine hydrochloride, 10 micrograms. prostaglandin E1, 0.2 mg. phentolamine mesylate and 0.075 mg. of atropine sulfate (group 1), or the same combination without atropine sulfate (group 2). Erectile response was evaluated subjectively and by intracorporeal pressure measurement. In group 1, 40 patients (35.1%) showed only tumescence, and 22 (19.3%) had poor erection. In group 2, 45 patients (39.5%) had tumescence and 17 (14.9%) poor erection. In both groups 52 patients (45.6%) had rigid erection. There was no statistically significant difference regarding erectile response and intracorporeal pressure. The addition of atropine sulfate did not improve results of the pharmacological erection test when combined with 50 mg. papverine hydrochloride, 10 micrograms, prostaglandin E1, and 0.2 mg. phentolamine mesylate.

Depending on the nature of the foreign body the diagnostic and management might be challenging. Aims. We report a case of an unusual magnetic self-inserted foreign body into the bladder for autoerotism and briefly discuss the diagnostic and therapeutic implications in this challenging situation. Methods. We describe all the steps we have used to adequately diagnose the problem, describe the foreign body and treatments for the patient. Related articles were found by utilizing the PubMed database and are summarized in this study. Results. The management approach must be planned according to the nature of the foreign body and should minimize bladder and urethral trauma. However, most of cases can be managed endoscopically.

To assess overactive bladder (OAB) prevalence, associated factors and implications in a young pop... more To assess overactive bladder (OAB) prevalence, associated factors and implications in a young population. An independent population-based study was carried out. A self-applicative questionnaire covering urinary symptoms, coping strategies, quality of life and treatment seeking behavior was developed. A total of 848 subjects between 15 and 55 years completed the questionnaire. The overall prevalence of OAB was 18.9%. Women were significantly more affected than men (p = 0.001). All age groups were equally affected (p = 0.152). Subjects with OAB reported significant impairment on household chores (p = 0.009), physical activities (p = 0.016), sleep (p < 0.001), work (p < 0.001), social life (p < 0.001) and sexual life (p < 0.001). To analyse whether the use of treatment-specific criteria for repeat biopsy maintains the usefulness of prostate-specific antigen (PSA) level for detecting high grade cancers. The REDUCE study was a randomized, double-blind, placebo-controlled investigation of whether dutasteride (0.5 mg/day) reduced the risk of biopsy-detectable prostate cancer in men with a previous negative biopsy. This chapter reviews the possible effectiveness of oral agents as an option for treatment of Peyr... more This chapter reviews the possible effectiveness of oral agents as an option for treatment of Peyronie’s disease (PD). We present the rationale for use of these drugs and base our recommendations on levels of published evidence and the report by the International Consultation on Urological Diseases. A definitive and ideal medical therapy for PD has yet to be established and will not be until we completely understand the trigger mechanism and the maintenance process of fibrosis in the tunica albuginea. Several oral substances have been proposed with promises of success; however, these agents appear to lack robust scientific support for their benefit. Until new evidence is available, the indications for oral treatment in PD are recent onset of disease, painful plaque, and unstable plaque. An early trial of inexpensive, safe, and well-tolerated oral therapy may be offered with limited enthusiasm for a positive outcome.

Renal malondialdehyde and serum creatinine levels significantly increased after renal ischemia -reperfusion. However, the pretreatment with allopurinol demonstrated a protector effect in these parameters. Renal ischemia -reperfusion provoked a significant renal damage in the operated group. Tubular atrophy and interstitial fibrosis were attenuated by allopurinol when given prior to the surgery. In our study, allopurinol had a strong tendency to exert a beneficial effect during renal ischemiareperfusion in uninephrectomized rats. D

Treatment can be achieved by prevention of priapism episodes with systemic therapy or by early intervention with intracavernosal self-injection of sympathomimetic agents. We describe a case of a young patient with recurrent idiopathic priapism who has used etilefrine self-injection for the past 10 years with good efficacy and libido and erectile function preservation. The object of this study is determination of sensitivity and specificity of serum FSH level and t... more The object of this study is determination of sensitivity and specificity of serum FSH level and testicle size in finding testicular sperm in azoospermic infertile men and comparison of these conventional markers with testicle biopsy results as a gold standard. Material and Methods: In this research, we studied 80 azoospermic infertile men who had been referred to Urology Clinic of Razi University Hospital in Rasht. Results: A testicle biopsy of 80 affected men, 53 azoospermic and 27 normal men with different levels of spermatogenesis was reported. In testicle biopsy of 53 azoospermic men, 41 patients were azoospermic on the basis of FSH level (sensitivityϭ77/3%) and of 27 patients who have spermatogenesis in biopsy of testicle, 27 patients were normal on the basis of serum FSH level (specificityϭ85/ 2%). Of 53 azoospermic men in biopsy, 50 patients distincted azoospermic on the basis of testicle size (sensitivityϭ94/4%), and of 27 patients with rows of spermatogenesis in testicle biopsy, 17 patients were normal on the basis of testicle size (specificityϭ63%). Conclusion: Measuring of serum FSH level and testicle size determination has acceptable sensitivity and specificity proportion of testicle biopsy that is gold standard for the evaluation of azoospermia. Of these two current conventional markers, testicle size has a greater sensitivity (94/4% versus 77/ 3%) and less specificity (63% versus of 85/ 2%) in proportion of FSH level.
