Introduction: This presentation is to discuss the
expansion and improvement of the well-being of the
people/resident and also how to enhance the
services of the community nursing team for more
productivity. The community nursing team provides
valuable care in or near people's homes. Their
clinical skills and knowledge enable them to help
residents/people manage their long-term health
conditions and achieve maximum independence. It
will cover:

FIRST MONTH TASK (CRUICAL)
POTENTIAL RISK
 The primary goal of this project is to design, implement,
and provide an expanded and improved community
nursing service in Greater Golden. This includes creating
a new service structure that prioritizes skill, budget
management, organizational structure, and referral
procedures. Also to improve community nursing services
by increasing efficiency, standardizing processes, and
facilitating effective collaboration, ensuring a developed
comprehensive and sustainable healthcare system that
meets the changing needs of the Greater Golden
community while improving residents' health and well-
being in the following areas;
2.To reduce hospital admissions by providing comprehensive care in the
community. This will relieve pressure on hospital resources and allow
patients to be discharged earlier.

3. Optimize resource allocation including; staff, equipment, and budget,


for
efficient and effective healthcare delivery in the region.

4. Improve staff satisfaction by addressing workload, stress, and morale,


the
project aims to improve employee satisfaction, retention, and
productivity.

5. Improve stakeholder satisfaction to meet the needs and expectations of


stakeholders, including patients, healthcare professionals, local
government, and community members, by providing high-quality and
accessible healthcare services.

1. Conduct a project kickoff meeting with key


stakeholders,

professionals, representatives from local

patient organizations. Use this opportunity to
present the
project, clarify objectives, assign roles and
responsibilities,
and subsequently engage with stakeholders to
gather
feedback through interviews, surveys, or focus
groups. This

 In the first month of the project, it is critical to lay the groundwork for
successful implementation. Here are some important tasks to
prioritize during the initial period:

1. Conduct a project kickoff meeting with key stakeholders, including

representatives from local government, and patient organizations.
Use this opportunity to present the project, clarify objectives,
assign roles and responsibilities, and subsequently engage with
stakeholders to gather feedback through interviews, surveys, or
focus groups. This iterative process ensures alignment with
stakeholder interests and enhances project effectiveness.
2. Carry out a comprehensive needs assessment in the Greater Golden
community to identify specific healthcare needs, including reviewing
existing data, holding community consultations, and examining
demographic and health trends. Simultaneously, inventory existing
resources such as staff, equipment, office space, and budgets to
determine their sufficiency and suitability to support the expanded services.

3. Conduct a thorough analysis to determine the required expertise for new


services such as mental health, physiotherapy, and maternity care, as well
as to assess staff training requirements for expanded services. Identify any
gaps in the current skill mix and staff knowledge or skills, and develop a
plan to fill them through recruitment, training, or collaboration with
external partners.
5. Standardize referral procedures for the five community
nursing
teams. Collaborate with stakeholders to standardize the
referral
process for the new service, ensuring efficiency and
consistency.

6. Create a communication plan to keep stakeholders


informed and
engaged during the project. Determine the channels and
frequency of
communication, as well as the main messages to convey.

7. Create a detailed project plan that includes timelines,


milestones, and
resource allocations.
 Several potential risks could have an impact on the project delivery.
Identifying and addressing these risks proactively is critical to ensuring
successful implementation. Here are some possible risks to consider:
1. Difficulties in recruiting and retaining qualified staff for expanded
services may cause project delays or have an impact on service
delivery, potentially leading to training delays for existing staff to
acquire new skills, affecting care quality and patient outcomes.
2. Budget Overruns: Unexpected costs, such as higher
salaries, equipment expenses, or office rentals, can cause
budget overruns and financial strain.

3. Operational Failures: Issues with equipment or systems


used for service delivery, such as electronic health
records or communication tools, may disrupt project
4. Change Management Issues: Staff and stakeholder
resistance to change, combined with inadequate change
management strategies, can hinder the adoption of new
practices and workflows.
5. Poor communication among project team members,
stakeholders, or external partners can cause
misunderstandings, delays in decision-making, and conflicts

Based on the highlighted risk, it is important we set out our


mitigation plans/strategies in order to have an effective project
delivery, meeting expected deadlines, ensuring a sustainable
and smooth-running of the project.
 Recommendations and solutions for the identified potential risks:

Staffing Challenges:

Mitigation: Develop comprehensive recruitment strategies to attract


qualified staff, including offering competitive salaries, providing
opportunities for professional development, and leveraging
recruitment agencies or networks.

Risk Plan: Communicate with HR departments and recruitment


agencies to address staffing shortages promptly. Implement cross-
training programs to ensure the team's flexibility and capacity.
Budget overruns:
Mitigation: Regularly review and monitor budgets to prevent cost
overruns. Implement strict financial controls and prioritize spending
according to project objectives and priorities.
Risk plan: Create a risk plan to identify cost-cutting or reallocation
opportunities. Communicate openly with stakeholders about budget
constraints and, if necessary, seek additional funding.
Operational failures:
Mitigation: Invest in reliable and up-to date technology infrastructure,
including redundancy and backup systems.
Risk plan: Create a risk plan to quickly troubleshoot and resolve
technical issues. Provide staffs with training and support to use
technology effectively and efficiently.
Change Management Issues:

Mitigation: Create a comprehensive change management plan, inclu

communication and collaboration among stakeholders.

dress any coordination issues immediately.
