EBP

The concept of EBP was initially described
in 1991
Since that time, many have expanded on this
concept and various health professions
have embraced this model as a means to
improve patient care. (Johnson 2008)

Evidence-based practice (EBP) is the integration


of best research evidence with clinical expertise
and patient values to facilitate clinical decision
making and the achievement of optimal patient
outcomes and quality of life. (Butcher, 2016)

Evidence-based practice (EBP) is a crucial
tool for delivering high-quality care in
numerous nursing specialties.
EBP enables nurses to make data-backed
solutions that incorporate clinical
expertise and current research into the
decision-making process
EBP combines clinical expertise, scientific
evidence, and the patient-professional
perspective to build well-rounded health
care strategies that can optimize care
delivery on a patient-by-patient basis.

 Nurses need a range of skills to effectively
implement and govern EBP strategies in a health
care environment.
 They must have strong leadership skills in creating
and proposing new solutions for patients.
 They must be able to analyze research, solve
problems, and make complex decisions based on
large quantities of evidence and data. Nurses must
also be adept at communicating their findings and
ideas to patients, families, and colleagues in the
health care facility.

Competence can broadly be defined as a concept that
incorporates a variety of domains including
knowledge, skills and attitudes. Health
professionals may demonstrate overall competence
in their relevant discipline via a four step process
including; (1) knowledge, (2) competence (specific to
the task), (3) performance, and (4) action. Apart
from knowledge, skills and attitudes, competence
also incorporates a health professional's problem
solving skills (e.g. ability to critically think and apply
clinical reasoning) and ability to work as a team
member and communicate effectively, both in a
written and verbal format. Assessing competences
can focus on any one of these domains.

Major challenges facing evidence-
based prescribing may be organized
into 3 categories:
(1) the availability of relevant evidence,
(2) the time and ability to
interpret evidence appropriately, and
(3) the translation of knowledge into
clinical practice.

EXAMPLE OF EBP USED IN THE CARE OF OLDER PERSONS

FALLS – IS UNPPLANNED DESCENT TO THE FLOOR THAT GIVES
INJURIES
 it is whether they result from physiological reasons (fainting) or
environmental reasons (slippery floor). Include assisted falls, such as when a
staff member attempts to minimize the impact of the fall.“

 According to Nurse.com, "It is estimated that between 700,000 and


1 million falls occur in U.S. hospitals annually costing real dollars, according to
a Center for Disease Control and Prevention report, 'Important Facts about
Falls.' It also is estimated that more than $31 billion was spent on non-fatal fall
injuries in 2015, reports the CDC. A hospital fall can add $14,000 and more
than six days to a hospital stay, for example."

 Due to this pressing and costly issue, many nurses are
turning to evidence-based practice for potential solutions for fall
prevention.

 By providing a safe hospital environment for patients,


completing thorough clinical assessments, improving both patient and
staff education and promoting open communication among team
members, the clinical staff can minimize the risk of patient falls and
improve patient safety.“

 "An interdisciplinary approach is key because the evidence


is clear: Fall-prevention programs that include only nurses aren't
effective. It takes a team to make a difference.“

 Evidence shows that one-third of reportable falls with injuries


in hospitalized older adults are linked to bathroom use; more than half
are associated with medications known to contribute to falls, such as
antianxiety and antipsychotic drugs. Also, about 40% of falls occur
within 30 minutes of an hourly rounding visit by healthcare providers."

 Nursing interventions can have significant impacts on patient care.
With their unique position on the front line, seeing risky patient behaviors first-
hand, nurses can craft care plans geared toward a patients with a high fall risk,
and prevent potentially life-threatening incidents.

 Planning these interventions as a multi-member healthcare team


promotes a safer environment for all patients, as evidenced by
AmericanNurseToday.com's observations. “Because falls have multifactorial
causes, an interprofessional team should collaborate in the comprehensive
assessment.
 Evidence-based practice is utilized in many aspects of healthcare.
Combined with previously researched methods proven to reduce incidence of
falls, EBP provides nurses and other healthcare providers with accessible
improvements to protect patients and prevent risk factors before an event.

The Filipino Nurses Role in EBP
There is no escaping the reality that nurses need to
fortify the nursing profession through the conduct of
research. We belong to a practice profession thus,
research is essential in developing and refining
knowledge that can be used to improve practice. We
need to use research findings to determine the best
way to deliver services and to ensure that the greatest
number of people receive services

This leads us further to a higher step of fortifying


nursing practice by moving on to an evidence-based
practice (EBP). EBP takes into consideration a
synthesis of evidence from multiple studies/evidences
and combines them with clinical expertise and patient
preferences and values.

Nurses Role in the practice of EBP for the elderly
Nurses and nurse scientists have been leaders in elder care and will:

 Play increasingly critical roles in addressing these challenges in the decades ahead.
 Conduct research that informs evidence-based interventions to promote health and
manage illness in various health care settings.
 Nursing science will continue to build the scientific evidence base for improved clinical
care and for improved quality of life for our aging population.
 Nurses provide the front line health care for older adults in a wide variety of settings,
including preventive care in primary care offices and in the community, acute care in
hospitals, and long-term care in nursing homes and assisted living facilities.
 Nurses and nurse scientists lead and participate in the interdisciplinary and
interprofessional teams necessary to understand and address the complex issues we face
in elder care, and to provide optimal patient care.(Grady, 2011)
