ENTEROVIRUS are distinguished
from RHINOVIRUS,
ENTEROVIRUS is stable at pH 3.
The capsids of ENTEROVIRUSES
are resistant to harsh
environmental conditions and
conditions in GI tract, which
facilitates their transmission by
fecal-oral route. They initiate GI
tract infections, but rarely cause
enteric disease.
COXSACKIEVIRUSES are divided
into 2 groups, COXSACKIEVIRUS
A & B, then subdivided into
numeric serotypes.
ECHOVIRUS
Enteric Cytopathic Human Orphan
RHINOVIRUS
Major cause of the common cold.
Sensitive to acidic pH & replicate
poorly at temperature above 330C.
At least 80% of the rhinoviruses and
several serotypes of coxsackievirus
bind to the intercellular adhesion
molecule-1 (ICAM-1) expressed on
epithelial cells, fibroblasts, and
endothelial cells.
Several coxsackieviruses, echoviruses, and
other enteroviruses bind to decay
accelerating factor (CD55), and
coxsackievirus B shares a receptor with
adenovirus. Poliovirus binds to a different
molecule (PVR/CD155) that is similar to
the receptor for HSV. The poliovirus
receptor is present on many different
human cells, but not all of these cells will
replicate the virus.
ENTEROVIRUS
Change in the receptor-binding
VP1 protein gene has occurred by
recombination with another
enterovirus. This development has
prompted a preference for use of
the inactivated polio vaccine.
Polioviruses are spread most
often during the summer and
autumn.

Pleconaril is available on a limited


basis. The drug inhibits
penetration of picornaviruses into
the cell. It must be administered
early in the course of the
infection.
POLIOVIRUS
1. ASYMPTOMATIC ILLNESS (90%)

2. ABORTIVE POLIOMYELITIS (minor illness) -


fever, headache, malaise, sore throat, vomiting

3. NONPARALYTIC, POLIOMYELITIS or ASEPTIC


MENINGITIS - progress into CNS and the
meninges (back pain, muscle spasm)

4. PARALYTIC POLIO (major illness) - virus


spreads from the blood to the anterior horn
cells of the spinal cord and motor cortex.
1. PARALYTIC POLIOMYELITIS - asymmetric flaccid
paralysis, no sensory loss, may involve only a few
muscle groups (e.g., one leg) or there may be
complete flaccid paralysis of all four extremities.

2. BULBAR POLIOMYELITIS - more severe, may involve


the muscles of the pharynx, vocal cords, and
respiration, and may result in death in 75% of
patients.

3. POSTPOLIO SYNDROME - Poliovirus is not present,


but the syndrome is believed to result from a loss of
neurons in the initially affected nerves.

RHINOVIRUS
RHINOVIRUSES are the most important cause of
the common cold and upper respiratory tract
infections. Such infections are self-limited,
however, and do not cause serious disease. More
than 100 serotypes of rhinovirus have been
identified. At least 80% of the rhinoviruses have a
common receptor that is also used by some of the
coxsackieviruses. This receptor has been identified
as ICAM-1, a member of the immunoglobulin
superfamily, which is expressed on epithelial,
fibroblast, and B-lymphoblastoid cells.
Unlike the ENTEROVIRUSES,
RHINOVIRUSES are unable to replicate in
the gastrointestinal tract. The
RHINOVIRUSES are labile to acidic pH.
Also, they grow best at 33° C, a feature
that contributes to their preference for
the cooler environment of the nasal
mucosa.

proteins block
apoptosis induced by

the virus or by T-cell
or cytokine (e.g.,
tumor necrosis factor
[TNF]-α) actions.
