Shinyanga of P.O. Box 28, Shinyanga do hereby Solemnly AFFIRM and state
as follows:-
1. That, I am the deponent in this Deed of Declaration and therefore

conversant with the facts I am about to depone.
2. That, I am the administratrix of the estate of the Late SELEMAN ABDI

ISLAM who was the lawful occupier of a piece of land located on PLOT
NO: 173 Block No. “Q” Located at MWOLEKA STREET, KAMBARAGE
WARD, SHINYANGA DISTRICT, SHINYANGA REGION.
3. That, the said property is in the name of Late SELEMAN ABDI ISLAM
who died on 17th day of June, 1992.
4. That, the following are the names of beneficiaries in the said estate
named SABAHA SELEMAN, ABDU SELEMAN, MUGENI SELEMAN, NASRA
SELEMAN,and NURU SELEMAN and I, myself who consented on
changing names and getting right of occupancy of the above
mentioned land.
5. That, I hereby declare that the said piece of land was occupied by the
late SELEMAN ABDI ISLAM who was my husband and now I as a legal
representative (administratrix of his estate) humbly request the
documents presented to the Commissioner For Lands for changing the
name and applying for certificate of right of occupancy be acted upon
as they are made bonafidely by me.
VERIFICATION
I, PILI JUMA, hereby verifying that all what is stated hereinabove in
paragraph 1, 2, 3, 4 and 5 is true to the best of my own knowledge and belief
and I make this solemn DECLARATION conscientiously believing the same
to be true and by virtue of Oaths and Statutory Declarations Act.

that we consent to the request presented by PERUCY RUBEN BUSUMABU
(who is now the as an Administratrix of estates of Late SELEMAN ABDI
ISLAM) to the Commissioner For Lands for changing the name and applying
for certificate of right of occupancy of the plot located at Plot No. 173
Block No. Q, at Mwoleka street at Kambarage ward, in Shinyanga
District at Shinyanga Region submitted for approval. we also understand
that by consenting to this application, we wave all rights toward the house
described above. We further understand that this consent is irrevocable.
