Medications: Cefuroxime 500 mg tab, 1tab BIDx5days ISMN 60 mg tab, tab OD Captopril 25 mg tab, 1/2tab 2Xa day Spironolactone 25mg tab, 1tab TID Bisopropol 2.5 mg tab, 1tab OD Bambuterol 10mg tab, OD Ferrous sulphate 1cap, OD Atorvastatin 80 mg tab, 1 tab OD
Rationale: To prevent infection To relieve angina attacks To decrease BP by inhibiting ACE. To reduce BP by dieresis, thus sparing K+ To decrease myocardial workload To treat asthma To treat anemia For lysis lipid build up on blood vessels
Exercise: 1. Passive ROM as tolerated such as: Neck flexion/extension Wrist flexion/extension Leg flexion/extension Ankle flexion/extension/rotation 2. Early ambulation as tolerated Treatment: Medication compliance. Health teachings: Instruct patient to increase dietary intake of high fiber foods such as: oatmeal, green leafy vegetables, red rice, and wheat bread. Instruct patient to have a low salt, low fat intake in the diet. Instruct patient to prevent strenuous activity such as: performing Valsalva maneuver. Instruct patient to visit the emergency room once angina doesnt subside when taken with 3 doses of nitroglycerine at home.

High salt diet can increase water retention and can contribute to an increased BP. High fat diet can contribute to more plaque on the blood vessels thus causing blockage. Follow up care: Instruct client to return to NMMC IM-OPD on august 25, 2011 at 2 pm for follow up check up after discharge. Diet: Low in fat: avoid food such as dairy products. Low salt diet: avoid foods that are high in salt such as dried fish, canned goods, and instant noodles. Include high fiber foods such as oatmeal red rice, wheat bread Spirituality: Encourage patient on attending regular Sunday mass. Confession with the priest every month.
