• The Client
Sheila A. Nitre was born on August 2, 1962. She is 60 years old and married to Sherwin M. Nitre. Sheila
and Sherwin are native of Tudok Mamot, Borongotan, Upi Maguindanao.
• The Family
The client Sheila lives in her house together with his husband Sherwin. Sherwin is working as a seasonal
farmer to maintain their daily needs.
• The Environment
The house of the client is located in Sitio Tudok Mamot, Borongotan, Upi Maguindanao. The client home
was made of mixed materials predominantly light materials, their source of water is from spring while
electricity is solar powered.
V. Assessment Statement
Shiela's husband Sherwin is the sole provider for the family, with the estimate 3,000 pesos monthly
income is barely enough to sustain their daily needs.
The family is currently facing financial hardship due to their minimal income but always have positive
outlooks to surpass it. The client and his husband still hopes for the better.
The client inclusion to Social Pension Program could help the family settle their every day expenses.

VII. Evaluation
Shiela A. Nitre , the client has no source of income considering her old age. And despite of his husband
hard work and perseverance they are still facing financial hardship. The minimal income of Sherwin
cannot even sustain their daily needs.
After the series of Intervention provided, the client will be successfully recipient of the Social Pension
Program and the grants from the said program will be a big help for the family.
V. Recommendation
The under design aocial work strongly recommends the follow up and immediate approval of social
pension application of the client Shiela A. Nitre for it will be a big help for the family.
