    AFFIDAVIT IN REGARD OF EMPLOYMENT

    SAID GRANTHON MWAKIPESILE

    I, SAID GRANTHON MWAKIPESILE, Christian of P.O BOX 3387, Dar es salaam do hereby
    take an oath and declares as follows;

    1. THAT, I am the deponent herein and therefore well conversant with what I am
    about to depose hereunder;

    2. THAT, I was an employee of BAHARI NURSERY AND PRIMARY SCHOOL (herein


    called “the employer”);

    3. THAT, I started working for the said employer from 01st day of June 2012 until 19th
    day of April 2022 when my employment contract came to an end;

    4. THAT, during my 10 years’ tenure as employee of “the employer” my finances have


    been entirely depending on my salary and now that the said employment has ended
    my finances are in shambles;

    5. THAT, I take this oath and state that since my employment has come to an end I
    have not been able to find another job whatsoever;

    6. THAT, I make this declaration in support of my application to collect my Social


    Security benefits that I and “the employer” contributed during my employment
    tenure;

    7. THAT, I made this declaration consciously believing the same to be true in


    accordance with the Oaths (Judicial Proceedings and Statutory Declarations) Act,
    Cap 34 R.E 2002;
