    es salaam do hereby take oath and state as follows;

    1. THAT, I am the deponent and therefore conversant with the facts I am


    about to depose.
    2. THAT, I am an aunt of the said PPPPPE OOOOO ORRRRRR.
    3. THAT, the said AKKKKE OOOOOO OIIIIIO was born on 21 st day of June
    1962 at Shirati, Rorya District in Mara region.

    4. THAT, I declare that I was immediately present after the birth of the said

    5. I further declare that the said AKKKKE OOOOOO OIIIIIO was born female
    who is the second born among six children.
