Thyroid-stimulating hormone (thyrotropin) is a glycoprotein released from the anterior pituitary. The synthesis and secretion of TSH is stimulated by thyrotropin releasing
hormone (TRH), a tripeptide produced by the hypothalamus. TSH stimulates the thyroid gland to produce thyroxine (T4) and triiodothyronine (T3) by binding to the TSH
receptor. The circulating free fractions of T4 and T3 in turn regulate the secretion of TSH by a negative feedback mechanism at the pituitary and the hypothalamus. 1 In
clinical practice, TSH assays can be used to detect thyroid dysfunctions (hypo- or hyperthyroidism), monitor thyroid hormone replacement therapy, guide the
management of subclinical thyroid disorder, and assess suppressive therapy in patients with follicular cell-derived thyroid cancer. 2- 4 When secondary hypo- or
hyperthyroidism is clinically suspected, TSH measurement aids in the diagnosis of pituitary diseases in conjunction with free T4 assays. 2, 3 Clinical guidelines recommend
using third generation assays to screen for overt or subclinical thyroid disorders, especially when presenting with low/suppressed TSH levels. The VITROS TSH3 assay meets
the criteria for a third generation assay with a high level of precision and a limit of quantification (LOQ) of 0.01 µIU/mL.
