This progress report is to be completed at the end of the semester. The form is designed to help field
placement supervisors provide feedback about the performance of students completing field placement.
Students and field placement supervisors are encouraged to set up a meeting a couple of weeks before
the end of the semester to complete the progress report. Students are expected to come prepared for
this meeting and the progress report is to be completed collaboratively between the student and field
supervisor. This form will become a part of the student’s record for this course and is a vital evaluation
component considered by faculty.

The following rating scale will be used in assessing the student’s performance in the areas contained in
the progress report.

 Advanced: The student requires no guidance and can coach, mentor, or train others to
perform the learning outcome successfully.
 Proficient: The student requires little to no guidance to perform the learning outcome
successfully.
 In Progress: The student requires help from others to perform the learning outcome
successfully.
 Not Applicable (N/A): The skill is not required or there is insufficient information to provide
an assessment.

Report meeting with their field placement supervisor.

1. Describe, with examples, how you developed therapeutic rapport/relationships with all the
children/young people at your placement? What challenges did you encounter and how did you
navigate those challenges?

2. Describe how you prepared for supervision with your supervisor. Include how you decided
what to talk about, how you prepared the agenda, and how you followed up with any
suggestions or feedback offered.

3. Describe 2 goals you have for the second half of your placement. Be sure to consider the
strategies you will use to achieve the goals and the indicators you will look for to know you
have met the goals.
