    JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS


    We, ANGELITA P. ADOL and BERNADETTE KRISTINE F. MACATO both of
    legal ages, Filipinos and residents of Davao City, after being sworn to law, depose
    and say THAT:

    1. We are the same ANGELITA P. ADOL and BERNADETTE KRISTINE F.


    MACATO who executed this affidavit;

    2. We know the late JONATHAN ODJENAR SUAZO born on August 9, 1996,


    and died on July 10, 2024 in Davao City , being our close family friend;

    3. Perusal of the Death Certificate of late JONATHAN ODJENAR SUAZO


    contained several erroneous entries to wit:

    a. In the Death Certificate of late JONATHAN ODJENAR SUAZO,


    entry as to his Birthdate was August 10, 1996, Instead of August
    09, 1996, which was true and correct;

    4. We hereby declare that the names:


    a. JONATHAN ODJINAR SUAZO as per Death Certificate, Born on
    August 10, 1996;
    b. JONATHAN ODJENAR SUAZO as per Birth Certificate, Born on August
    09, 1996;
    Refers to one and the same person

    5. We are fully aware of the above-stated facts because we are close friends
    and neighbors of JONATHAN ODJENAR SUAZO and are intimately
    acquainted with each other;

    6. We are not related by consanguinity or affinity to JONATHAN ODJENAR


    SUAZO nor are we financially interested in his case;

    7. That we are executing this affidavit to attest to the truth and to inform the
    proper authorities concerned and for whatever legal purpose this may serve.
