    AFFIDAVIT OF DISCREPANCY
    (ONE AND THE SAME PERSON)

    I, DAISY JUMAPIT DIMALUAN, Filipino, married to ALLAN A. DIMALUAN, of


    legal age, with address at 124 Zone Ahos, 6.5 Paknaan, Mandaue City, after having
    been duly sworn to in accordance with law hereby depose and say that:

    1. The Certificate of my Live Birth indicates the name DAISY C. MAPIT, born on
    March 7, 1968, in Pualas, Tubod, Lanao Del Norte;

    2. On the other hand, my other documents like school records, SSS, Philhealth,
    among others, and I, myself, have been using the name DAISY C. JUMAPIT,
    before I was married;

    3. Both names and my other documents pertain to one and the same person;

    4. I am executing this affidavit to attest to the truth and for whatever legal
    purposes it may serve.

    IN WITNESS WHEREOF, I hereby affix my signature this 10 th day of


    November 2018, at Mandaue City, Cebu, Philippines.
