I – KNOWLEDGE (20%)
1. Define Fundic Height, Leopold’s Maneuver and FHT.
2. State the purpose.
3. Identify materials/equipment needed.
II – SKILLS/ABILITY (70%)
1. Wash hands. Verify patient’s identity.
2. Explain the procedure to the client.
3. Instruct client to empty her bladder first.
4. Drape properly to provide privacy. Expose only the part to be assessed.
5. Place woman in dorsal recumbent position, supine with knees flexed to relax abdominal
muscles. Place a small pillow under the head for comfort.
6. Warms hands by rubbing together. (Cold hands can stimulate uterine contractions).
7. Place a tape measure midline at the top of the patient’s symphysis pubis.
8. Smooth the tape along the patient’s abdomen until the fundus of the uterus is palpated.
9. Determine the number of centimeters from the symphysis pubis to the uterine fundus.
7. 1st maneuver - Fundal Grip: Face the head part of the client. Using both hands, feel for the
fetal part lying in the fundus. Palpate the upper abdomen with pads of the fingers/palms of
both hands.
8. 2nd maneuver - Umbilical Grip: Still facing the head part of the client, place the palms of both
hands on either side of the abdomen and make gentle and deep pressure.
9. One hand is used to steady the uterus on one side of the abdomen while the other hand
explores the other side of the uterus.
10. Repeat the maneuver on the other side. This maneuver will give a more accurate picture of
the fetal orientation.
11. 3rd maneuver - Pawlik’s Grip: Still facing the client, using thumb and finger, grasp the lower
portion of the abdomen above symphysis pubis, press in slightly and make gentle movements
back and forth.
12. The presenting part is engaged if it is not movable. It is not yet engaged if it is still movable.
13. 4th maneuver – Pelvic Grip: This is done facing the women’s feet.
14. Using both hands. Palpate fetal head pressing downward about 2 inches above the inguinal
ligament.
15. Determine the fetal attitude.
16. If the uterus is not contracting, apply water-based lubricant and place the bell of the
stethoscope over the quadrant of the mother’s abdomen where the fetal back is located.
17. Listen for and count the fetal heart tone in one full minute. Take note of the rate, regularity,
strengths and any deviation of the fetal heart tone.
18. Make the client comfortable.
19. Record/Document.
III - ATTITUDE (10%)
1. Demonstrate preparedness, readiness and confidence in the performance of the
procedure.
2. Accepts corrections/suggestions and shows willingness to improve performance.
3. Answers questions politely and tactfully.
4. Observe proper decorum and behave accordingly as a student nurse.
