Rapid Risk Screening Exercise for specified dams in India:

I. Reports required:
For DRIP Dams
 DSRP Report
 Project Screening Template (PST)
 Latest Inspection Report
 Emergency Action Plan (EAP)
For Non- DRIP Dams
 Salient features
 Latest Inspection Report
 Geology Report
 Cross-section including Longitudinal cross-section
 Log-book of instrumentation data

II. Information required:


For Calculation of Fragility Index
A. Technical Characteristics:
1. Age: Information on Commissioning year and if there was any distress during first
filling event?
2. Inflow design flood: Information on revised design flood and highest recorded
inflow.
3. Seismic design: Seismic zone for area and if Site specific Seismic studies are
available?
4. Landslides, GLOFs, LDOFs, and debris flow: Data to be Collected: • Geology of
Region and the potential for these formations • Recent Historical Evidence of
formations • Geotechnical Studies and Investigations (Example: Are the studies been
carried out that demonstrate a low potential for major landslides and/or debris flows to
occur? (Information on Recent and Past Sedimentation studies been done) •
Degree of Mitigation Measures in place for formations.
5. Length: Total length of Embankment, Concrete & Masonry component.
6. Conduits: In embankment, abutment & overburden foundation.
7. Filter: Are the filters well designed? Are the drawings available and properties of the
filter known? Distress condition of filters? Is there a way to inspect the existing filters?
Monitoring of Discharge through filter.
8. Foundation: Geotechnical Investigations and analyses of foundations: material type,
gradation, density, shear strength, and permeability • Geologic presence of problematic
formations (dispersive soils, karst, glacial deposits, highly fractured or jointed rock,
slip zones, landslides) • Adequacy of designed mitigation measures (filters, drains,
berms, relief wells, weirs, cutoffs, grouting, stabilizations, and other foundation
improvements.)
B. Existing Conditions:

• Adequacy of design compared to current standards
• Resilience of dam based on type and foundation materials
• Performance during earthquakes
• Adequacy of defensive measures
• Site specific seismicity changes

2. Installed Flow Discharge Equipment: Data to be Collected:


• Discharge capacity as percentage of design flood considering all: • Service
spillways • Auxiliary spillways • Emergency spillways • Low Level outlets •
Powerhouse under some conditions

3. Actual Flow Discharge Equipment: Data to be Collected on Equipment: • Testing and


maintenance compliance with operations manual • Condition/Operability •
Performance History

4. Back Up Power: Data to be Collected: • Currently On-site availability • Power can be


arranged • Back up power not available?

5. Access to Site: Data to be Collected: • Condition of the access road to the site • History
of accessibility to site • Potential for the road to be impassable due to flooding by
releases, fires, or bridges • Location and availability of remote staff to respond.

6. Concrete Gravity dam: Data to be Collected: •Regarding Observed distress in:


Cracking, Foundation drains, Concrete deterioration, Wet surfaces, Leaching,
Movement/misalignment, Rock formations, anchors failed due to deterioration?

7. Spillway Structures: Data collected on damages seen as in: Displacement and offsets,
Cracking, Joints, Erosion, Energy dissipators.

8. Masonry dam: Nature of mortared joints, Seepage existing through dam body?
Leaching, movement, settlement of dam if any? Cracks and expelling out of masonry
elements? Anchor conditions? Information on instrumentation trends?

9. Embankment, Abutment and Foundation : If concrete or masonry dams founded on


overburden or other problematic natural materials: • Data to be Collected: • Movement
distress from settlement and global instability, (cracking, depressions, bulging, erosion,
slides, riprap) • Internal erosion distress from backward erosion & piping, concentrated
leak erosion, and suffusion (boils, sinks, seepage, vegetation, discharge and
sedimentation, burrows, and cavities) • Instrumentation Trends • Geologic hazards •
Amount of Freeboard at normal pool
C. Safety Plans:
1. Documentation: Data to be Collected: • Catalogue of key documentation: • Original
plans and specification • Construction Records • Design drawings and analyses •
Foundation Reports • Planning Documents • Operation and System Design

2. Operation and Maintenance manual: Data to be Collected: • General Information •


Roles, Responsibility, and Governance • O&M Work Schedules • General and specific
Operational Plans (water control) • Inspection Schedules • Maintenance Plan •
Instrumentation Plan • The degree to which all of these documents are implemented?

3. Emergency Action Plan: Data to be Collected: • Copies of Plans which describe


who, how, and when the owner communicates during dam incidents and failures •
Data on how the plan is exercised and updated. • Information on how the plan is
communicated to the public. • Plans which describe how remedial actions to reduce
risk are implemented, including procurement of services.
4. Organization, Staffing and Competence: Data to be Collected: • Dam Safety
Organization Structure and Size Relative to What Should Exist • Designation of Dam
Safety Leader and Competent Staff with Training and Experience • Degree of
Professionalization.

5. Data on Safety Inspections, Reporting, and Monitoring: Data to be Collected: •


Frequency, type, and nature of inspections and assessments • Frequency, type and
nature of instrumentation monitoring • Tracking, reporting, and management of
findings and recommendations • Training of Personnel on Dam Safety

6. Dam safety reports with analysis and interpretation: Data to be Collected: •


Reporting in accordance with policies, guidelines, and laws • Completeness of
Reporting • Tracking of Reporting.

7. Follow-up Actions: Data to be Collected: • Number of Actions vs. Recommendations

8. River system operation: Is dam a part of multiple dam system? Is there any integrated
flood management plan? Information on inflow forecast? Is control room accessible?
For Calculation of Potential Impacts:
1. Population at risk: Data to be Collected: • Population at Risk and Methodology Used.
(From EAP or Dam break studies)

2. Environmental Impacts: Data to be Collected in Inundation Areas: • Number and


density of threatened or endangered species • Morphological impacts • Presence and
size of protected natural areas • Habitat and restoration feasibility (comments on loss
of fish and/or wildlife habitat with high capability of natural restoration) • Presence
and sensitivity of hazardous facilities.

3. Socio-Economic Impacts: Downstream Data to be Collected: • Concentration of


residential, agricultural, industrial, and commercial development • Civil Public
infrastructure • Interruptions of water commerce • Public Buildings/Facilities • Nuclear
Facilities • Potential for Cascading failures of downstream dams.
