    HOW TO APPLY FOR MILK BOOTH SCHEME

    Sign Up – For Sign up user will enter their full name, email id and mobile no to get registered.

    After Sign Up this popup will comes with message that, User registered successfully, and password will
    be sent to their registered email id.
    After sign up, user will receive their Password on their registered email id.
    Forget Password – User will enter registered email and mobile for getting existing password which will
    be sent to user’s registered emailed.
    Sign In – User will enter registered email id and password received on email for login.
    Dashboard – After successfully login, user will get this Dashboard.
    There are four options on dashboard.

    1. Download/Activate Form (Milk Booth)


    User will get this screen for making payment.

    On selecting option ICICI Bank and on clicking Accept Instruction Terms and Condition and
    Proceed, user will redirect to payment page where amount will come automatically. User will
    provide account credentials required for payment and payment will complete.

    After Payment, user can download the Brochure.


    2. Apply for Milk Booth
    Without payment on clicking option Apply for Milk Booth, user will get below message.
    Application Form – When user is able to download brochure and on clicking this option, user
    will get application form. User will fill all the mandatory fields marked with red star symbols.
    Document Upload – After submitting form successfully, user will be asked to upload documents.
    Acknowledgement Slip – User will get acknowledgement slip after successfully uploading the
    documents
    Payment Option – User will come on this page for payment.

    Payment Gateway – User will come on this page with pre filled amount, and after entering
    account detail user will successfully pay the said amount.
