• Review P3 (Preparation Planning Procedures) and JHA with crew
Preparation
• Assess location and conditions*
• Complete and review STA (Safety Task Assessment) daily
Proper layout • Review plans and specs; discuss with owner rep.
Damage to existing buried • Notify Dig Safe and all non-member utilities prior to start
utility lines • Mark all other private utilities; maintain markings
Mark and locate all • Follow 18” rule for locating utilities: Use hand tools to dig within 18” of
existing utilities the marked line until the utility is exposed.
Overhead electrical or • Follow the 10’ overhead clearance rule
telephone lines
Being hit or struck by • Maintain eye contact with excavator operator
machine or excavation • Maintain safe distance from excavator and bucket at all times
Excavate trench for
• All personnel should be out of trench when backfilling

Person falling or tripping • Good housekeeping around excavation
into trench or test pit • Berm and barricade excavation
• Keep non-essential personnel away from the hole
• Use a quality ladder of sufficient size to access excavations more than 4’ deep
• Never walk on edges or spreaders of trench boxes
Hazardous atmosphere • Use air monitor to test air quality in trenches and confined spaces where
sewer gas or other hazardous gases could exist
• Use air monitor when using gas powered tools in trench
Exposure to high levels of • Use rubber gloves
Excavate trench for bacteria while working • Wash hands before eating

ends
Cut by pipe saw • Personnel must be properly trained before using pipe saw
• Proper set up of work area to safely cut pipe
Cuts from pipe • Wear gloves and face protection
