DESIGN AND IMPLEMENTATION OF E-DIAGNOSIS SYSTEM

DECLARATION

i
CERTIFICATION

ii
This is to certify that VICTOR of Computer Science Department carried out this project under the

supervision of Mr., in the partial fulfillment of the award of Bacholr degre in Computer Science

DEDICATION

iii
This project is dedicated to Almighty God for his grace, love, mercy and protection over me

from the beginning of my program to the end in this polytechnic. This project is also specially

dedicate to my parents Mr and Mrs.Rabiu, who have always contributed morally, financially and

who always been there for me before and during the course of this project.

ACKNOWLEDGEMENT

iv
My profound gratitude first goes to my supervisor Mrs. Owadoye for his incomparable assistance,

constructive criticism, guidance and patience going through the write up at any time with all

pleasure. May Almighty God continue to strengthen and bless you in all your undertakings (Amen).

My gratitude also goes to those who in a way have contributed to this great success of my study, like

Mr. Olapade, Mr. Alade, Mr. Yekini, Mr. John and others that cannot be mentioned.

ABSTRACT

v
Health is essential for everyone. Health can’t be bought by anything. Health is important no matter

what country we are from or what race we are, or what age or gender we are. Some important details

like the patient’s allergy and medical history will be overlooked by the doctors when they

diagnosing. The procedure for diagnosing also consume a big amount of time although the patients

comes with a minor problem that they can settle it by themselves if they have the right help. This

paper discusses about e-Diagnosis system. This system is split into these main parts that comprise of

Patient’s Registration and Administration, Diagnosis, Appointment, Prescription, Health Monitor

and Tips. The programming languages used in developing the system are, Hypertext markup

language (HTML) in conjunction with the Hypertext PreProcessor (PHP) scripting language, JQuery

and Information used by the system was stored on the internet with the use of the MySQL database.

This project is design not to replace medical doctors or medical laboratories but rather to assist them

in accelerating in their analytical profession which will go a long way to eliminate the trial and error

method of treatment which the doctors usually perform while waiting for the test result of a

particular patient to be release which can also be detrimental to the patient. In view of this, a careful

study and understanding of this research will assist users, medical doctors and lab scientist.
