underwriting your policy to charge the insurance premium (including applicable taxes and fees) for this policy to your credit card
account. Your first payment was charged to your credit card upon completion of your purchase and your next payment will be
charged, monthly, based on your selected payment frequency.
• You agree that there will be no pre-notification period for any changes to the amount or date of your payments.
Note: Changes to your insurance coverage may result in changes to the monthly premium charged to the associated credit
card and/or the date upon which the associated credit card is charged.
• You also agree that this authorization will remain in effect until you cancel it. You may cancel this Agreement by advising us

Ontario Automobile Policy (OAP 1). In return for the premium charged and the statements contained in the Application, the
contract provides the coverage outlined in this Certificate. You only have a particular coverage for a specific automobile if this
Certificate shows a premium for it, or shows the coverage is provided at no cost. All other terms of the Policy remain the same
unless stated otherwise in this Certificate. Your Insurer will provide you with a copy of the Policy if you request it. This
Certificate is only valid if it is signed by an authorized representative of the Insurer.
For purposes of the Insurance Companies Act (Canada), this document was issued in the course of the Insurer’s insurance
