neurologic patient1
• Assists with home environment set up
• Speaks to family regarding finances, support, and the situation regarding the patient’s recent
neurological insult
• Confirms discharge planning ahead of time so that the patient can transfer out of acute care
• Major patient advocate during interdisciplinary meetings
• Provide patient and family with resources

• A medical social worker can work with all diagnoses {CVA, TBI, SCI, etc.}

– Bachelor’s degree in Social Work
– Master’s degree in Social Work at an accredited program

• Licensing following MSW


– Limited Licensed Masters Social Work
– Compete 4000 hours under a Licensed Master of Social Work (LMSW)

Tests and measures done with

• Medical social workers have no specific test/measure completed with patients
• However, very comprehensive questioning is done with neurologic patients in acute inpatient settings
when discharge is a topic of conversation
• Questions a social worker may ask:
– Home set up
– Family support
– Hobbies/recreational activities
– Full patient history {specialists, medications, surgical, etc.}
– Financial

• Social workers can provide the following resources to both the patient and their family/support
members in the hospital setting
– Counseling resources {psychology referrals, support groups, }
– Financial resources (food banks, signing up for government resources, contact insurance agencies}
– Recommendations on facilities for discharge and educate family on options {IPR, Nursing Home, AFC, etc.}

• Social workers are an essential patient and family advocate in any medical setting!

– Climate change {Not yet introduced to Congress}

Physical Therapists and social workers
• Support
– Interdisciplinary teams- advocating for patients together and using both disciplines knowledge to support a discharge
recommendation

• Overlap
– Inpatient acute hospitals
– Inpatient rehab
– Nursing homes
• Co-treatment
– Not necessarily at the same time
– However, both disciplines may refer for the other depending on the situation!
