    No change in the formulation, labeling and commercial presentation of this product shall
    be made at any time during the effectivity of this registration without prior written approval of
    this Office.

    This marketing authorization is subject to suspension, cancellation or recall should any


    violation of Republic Act 9711, Presidential Decree B81and/or regulations issued thereunder
    involving the product be committed.

    CONDITIONS:

    trA- Complete name and address (including street name) of the manufacturer / importer / trader must appear on the
    label.
    DB- Registration (HSR) Number must appear on the label as HSR-5860.
    trc- Net content expressed in metric unit must appear on the label.
    DD- DIRECTION FOR USE, must appear on the label.
    trE- The following precautionary statements must appear on the label:
    tr 1. Keep Out ofReach ofChildren.
    tr 2. CAUTION: Hannful if swailowed. If accidentally swallowed, do not induce vorniting. Avoid
    contact with eyes and prolonged contact with skin. lf splashed on eyes, flush with cold water
    immediately and call a doctor. If spilled on skin, clothing, or floor, wash off with water. Keep
    out of reach of children.
    tr 3. CAUTION: Contents under pressure. Do not puncture or incinerate. Do not spray into open
    flame. Do not expose to excessive heat or to temperature above 30 degrees C (120 degrees F).
    Keep out ofreach ofchildren.
    ! 4. CAUTION: Do not use together with other bowl cleaners or chemicals. Avoid contact with
    eyes or skin. Not to be taken intemally. Keep out of reach of children.
    n 5. CAUTION: Vapour may cause drowsiness or nasal irritation. Avoid contact with or prolonged
    contact with skin.
    tr 6. CAUTION: Repeated contact may irritate sensitive skin. Wipe skin with cloth and wash well
    with soap and water. Do not take intemally. Keep out of reach of children.
    u 7. CAUTION: Eye and skin irritant. Bonds skin instantly, contains cyanoacrylate. Avoid contact
    with skin and eyes. If eye or mouth contacts occur, hold eyelid or mouth open and flush with
    water only. Get medical attention. If skin bonding occurs, apply solvent (acetone or nail polish
    remover) to contact area and gently peel or role skin aparl. (Do not pull). Use with adequate
    ventilation. Keep out of reach of children.
    D 8. CAUTION: Cornbustible mixture. Keep away from excessive heat, or open flame. Keep out of
    reach of children.
    tr 9. CAUTION: Flamrnable. Keep away from heat, sparks and open flame. Avoid prolonged or
    of vapors and prolonged or repeated contact with skin. Use with adequate
    repeated breathing
    ventilation. Do not take intemally. Wash hands thoroughly before srnoking, eating, or using.
    Keep container closed.
    trF- Instruction for first aid treatments when necessary or appropriate must appear on the label.
    trG- The common or usual name of the chemical name of the hazardous substance must appear on the label.
    lH- Submit final printed label and comnrercial sample within six (6) months from receipt of this initial/renewal
    HSR.
    rr- In case there are gaps found upon the bio-efficacy data, marketing authorization will be tbr outright
    cancellation.

    SPECIAL CONDITION:

    The registration of the product herein granted shall not be interpreted or construed as an endorsement or
    representation by FDA, that Registrant has the right or privilege to the use of the name or brand so registered.
    Registrant hereby agree and affirm to indemniflz and/or hold FDA fiee and harmless against any and ail third-
    party claims on in{iingement of patent, trademark or intellectual property rights arising fiom the registration
    ofproduct(s) listed on the other side hereof.
