    (RE: House Renovation)

    I, ERLOREN E. MATEO, Filipino citizen, of legal age, widow of the late


    Msgt. Edgardo H. Mateo, with residence and postal address at P18-12 10th corner 5th
    St., Barangay 183, Villamor, Pasay City, after duly sworn to in accordance with law,
    depose and say:

    That I am the widow of the late, Msgt. Edgardo H. Mateo, who was the owner
    of the house and lot located at P18-12 10th corner 5th St., Barangay 183, Villamor,
    Pasay City;

    That I am intending to renovate my abovementioned residence with the


    measurement of 6 meters by 6 meters starting upon approval of the issuance of
    Barangay Certificate / Clearance for renovation;

    That I am executing this affidavit to inform the proper authority of the truth of
    the foregoing facts for whatever legal purpose it may serve.

    In Witness Whereof, I affixed my signature this 20th day of October 2022 in


    Parañaque City, Metro Manila, Philippines.
