WHAT IS SKIN PREP?
Skin preparation, also known as preoperative skin preparation or skin antisepsis, is a crucial step
in the preoperative process to reduce the risk of surgical site infections (SSIs). The goal of skin
preparation is to eliminate or reduce the microbial load on the patient's skin at and around the
surgical site. SSIs can occur when microorganisms from the skin's surface enter the surgical
incision during an operation, leading to infections that can be both serious and costly.

TYPES

1. Alcohol-Based Skin Prep:


Purpose:
Provides antimicrobial activity and aids in the reduction of skin flora.
Procedure:
 Clean the intended surgical site with an alcohol-based solution applied unto a cotton
ball or operating sponge (e.g., chlorhexidine-alcohol or povidone-iodine-alcohol).
 Follow the recommended drying time or dry with a clean and dry cotton ball
 Ensure complete coverage of the surgical site.

2. Povidone-Iodine (Betadine)
Purpose:
Broad-spectrum antimicrobial activity.
Procedure:
 Apply povidone-iodine solution to the surgical site or onto the operating sponge,
using an inner to outer method, observing sterility
 Repeat for 2-3 times or according to institutions policies
 Rinse if indicated by institutions policies
 Allow it to dry completely.
3. Chlorhexidine Gluconate:
Purpose:
Antiseptic with persistent antimicrobial activity.
Procedure:
 Apply chlorhexidine solution to the surgical site or onto the operating sponge,
using an inner to outer method, observing sterility
 Repeat for 2-3 times or according to institutions policies
 Allow it to dry completely.

4. Iodophor-Based Preps:
Purpose:
Antiseptic action with persistent activity.
Procedure:
 Apply an iodophor-based solution to the surgical site or onto the operating
sponge, using an inner to outer method, observing sterility
 Repeat for 2-3 times or according to institutions policies
 Follow the recommended drying time.
5. Prepackaged Antiseptic Swabs:
Purpose:
Convenience for small or targeted areas.
Procedure:
 Use prepackaged antiseptic swabs containing chlorhexidine or povidoneiodine.
 Apply to the surgical site in a systematic

6. Hair Removal:
Purpose:
Remove hair to reduce the risk of infection.
Procedure:
 Clip hair if necessary; avoid shaving, as it can increase the risk of SSIs.
 Remove loose hair from the surgical site.
HOW ITS DONE
GENERAL STEPS FOR SKIN PREPARATION:
1. Hand Hygiene:
Wash hands thoroughly with soap and water or use an alcohol-based hand sanitizer.

Scrubbing Technique:
Purpose:
Removes dirt, oil, and transient microorganisms from the skin.
Procedure:
• Wet hands and forearms.
• Apply an antimicrobial soap or surgical scrub solution according to your facility's
protocol.
• Scrub hands and forearms for the recommended duration (usually 3-5 minutes).
• Pay attention to nails, cuticles, and between fingers.
• Rinse thoroughly and dry hands using a sterile towel.

2. Personal Protective Equipment (PPE):


Wear appropriate PPE, including sterile gloves and a gown.
3. Patient Positioning:
Ensure the patient is in a comfortable and accessible position for the procedure.

SKIN PREP FOR DIFFERENT BODY PARTS


1. Abdomen and Groin:
 Clip hair if necessary; avoid shaving.
 Clean the skin using an antimicrobial soap or scrub as per institutional protocol.
 Apply an antiseptic solution (e.g., povidone-iodine or chlorhexidine) in a concentric
fashion, starting from the incision site and moving outward.
 Allow the antiseptic solution to dry completely before draping the surgical area.

2. Upper Extremities (Arms):


 Clean the skin using an antimicrobial soap or scrub.
 Apply an antiseptic solution, ensuring complete coverage.
 Pay attention to the axilla and the area around the shoulder joint if applicable.
 Allow the solution to dry before proceeding with the surgery.
3. Lower Extremities (Legs):
 Clip hair if needed; avoid shaving.
 Clean the skin using an antimicrobial soap or scrub.
 Apply an antiseptic solution, covering the entire leg, including the foot if necessary.
 Allow the solution to dry before draping

4. Chest and Back:


 Clean the skin using an antimicrobial soap or scrub.
 Apply an antiseptic solution, ensuring coverage over the chest and back.
 Pay attention to areas around the breasts if applicable.
 Allow the solution to dry before draping.

5. Head and Neck:


 Clean the skin using an antimicrobial soap or scrub.
 Apply an antiseptic solution, covering the surgical site and surrounding areas.
 Pay attention to hairline and ears if applicable.
 Allow the solution to dry before draping.
6. Perineal Area:
 Clean the perineal area with an antimicrobial soap or scrub.
 Apply an antiseptic solution, ensuring thorough coverage.
 Pay attention to the folds and creases in the perineal region.
 Allow the solution to dry before proceeding.

Always adhere to the specific guidelines provided by your institution or surgical team.
Preoperative skin preparation is a collaborative effort involving the entire surgical team to ensure
aseptic conditions and minimize the risk of postoperative infections.
