LEADERSHIP AWARD

AWARD GUIDELINES

The FEU Alabang Leadership Award gives recognition to the holistic achievement of a student leader.
The recipient of this award shall embody the vision, mission and core values of FEU Alabang and his
Organization/s.

The award is given by the Student Services Office in recognition of deserving students who have shown
exemplary performance in extracurricular activities, co - curricular activities, community involvement
and socio-civic service.

QUALIFICATIONS

1. He/She must be a bona fide graduating student of FEU Alabang in the current school year with
a residency in the College for at least six terms.

2. He/She must be at least a member of a Recognized Student Organization of the College.

3. He/She must not have incurred a failing grade in any of his/her subjects.

4. He/She should possess good moral character and must not have committed any major offense.
5. He/She must have garnered at least 80 points in the criteria set by Student Services.

REQUIREMENTS

1. Personal Information Profile with 2”x 2” picture


2. Accomplished Summary of Achievement Profiles.
3. Clearance signed by the Student Activities and Development Office, Guidance Unit, and
Discipline Unit
4. Supporting Documents (i.e. photocopied Certificates or photo of plaques or medals)

SUMMARY OF ACHIEVEMENT PROFILE


1. Activities attended or participated and awards received must be arranged in chronological
order.
2. Date column must be filled up with month/date/year (ex. 05-11-2017)
3. Legend for the Level column:
i. I –International
ii. N – National
iii. R – Regional
iv. C - City
v. S – School

4. You may add more rows if needed. Blank rows however, should be deleted.
5. For the Honors/ Awards Received Profile, you shall include all academic honors and awards
received from academic institutions, organizations and contests. You shall also include
awards or distinctions in leadership, student service, performing arts, journalism, civic, etc.
Honors and awards must be arranged in chronological order from most recent to earliest.
6. For the Position in Organization Profile, you shall include all your positions in
affiliated organizations (School and/or Civic Organizations) during your stay in FEU
Alabang.
7. Indicate all your Organized Activities in its corresponding profile. It shall include
organized General Assemblies, Seminars/Conferences/Convention, Contests, Programs,
Projects, academic or school related. You shall also indicate your participation in
programs (ex. Chairman of Logistics, Host).
8. In the Attendance or Participation in Activities Profile, indicate all attended or
participated off- campus or in-campus activities other than organized by your organization
and Student Activities. You shall also indicate your participation in programs (ex.
Participant, Host, Intermission Number).
9. The Attendance or Participation in Student Activities-Organized Activities Profile shall
include your attendance and/or participation in Student Activities (ex. Seminars by
Guidance Unit, LTS of SADO). Participation in CESU activities however, shall be included
in the Community Involvement Profile.
10. For the Community Involvement Profile, indicate all your civic activities and community
involvements done voluntarily or independent of the school such as work done in barangay
or done in connection with the school's extension services.
11. The supporting document chapter must also be arranged in sequential order of the summary
of profile (Academic, Leadership, and Community Involvement).
12. Place all documents in a LONG DARK GREEN CLEAR BOOK with your complete name,
student number, course, contact number, and email address.
13. Accomplished application forms and supporting documents should be submitted on SADO on
the said deadline.

THE SELECTION PROCESS

I. SUBMISSION OF APPLICATION

A. Applicant must submit complete requirements within a set deadline to the Student Activities
and
Development Office (SADO).
B. Place all documents in a LONG DARK GREEN CLEAR BOOK with the applicant’s complete
name, student number, course, contact number, and email address.

II. PRE-SCREENING BY SADO

A. SADO shall thoroughly review the application in accordance with the Award Guidelines and
Requirements.
B. Applicants may be called for an interview by SADO.
C. All applicants who garnered 80 points in the pre-screening by SADO will be forwarded and
endorsed to the Honors and Awards Committee for further review, deliberation and
approval.

III. FINAL SCREENING BY HONORS AND AWARDS COMMITTEE

A. All endorsed applicants shall be reviewed and deliberated by the Honors and Awards
Committee which will be composed of the Academic and Program Directors, headed by
the Registrar.
B. All finalists may be called for a final interview by the Committee for further deliberation.
C. The Committee shall deliberate en banc as to who shall be awarded the FEU Alabang
Leadership Award.
D. Conferment of the award shall take place during the graduation rites.
