LIVER BIOPSY

Instruction: Below is the grading scale which will be used in rating your performance.
5 - Excellent - 95-100
4 - Very Satisfactory - 88-94
3 - Satisfactory - 82-87
2 - Fair - 76-81
1 - Needs Practice - 70-75

PURPOSES
 Diagnose a liver problem that can't be otherwise identified
 Obtain a sample of tissue from an abnormality found by an imaging study
 Determine the severity of liver disease — a process called staging
 Help develop treatment plans based on the liver's condition
 Determine how well treatment for liver disease is working
 Monitor the liver after a liver transplant
EQUIPMENT
 Liver biopsy tray (contains needles, scalpel, specimen tubes, etc.)
 Sterile gloves
 Antiseptic solution
 Local anesthetic
 Sterile dressing
 Sphygmomanometer to monitor BP
IMPLEMENTATION
Prior to performing the procedure, introduce self and verify the client’s identity using
agency protocol. Explain to the client what you are going to do, why it is necessary, and how he
or she can participate. Provide for a means of communication, such as eye blinking or raising a
finger, to indicate pain or distress. Follow through by carefully observing the client throughout
the procedure, offering periodic eye contact, caring touch, and verbal reassurance.

PERFORMANCE

PRE PROCEDURE 5 4 3 2 1
1.Ascertain that results of coagulation tests (prothrombin time, partial
thromboplastin time, and platelet count) are available and that
compatible donor blood is available.
2. Check for signed consent; confirm that informed consent has been
provided.
3. Measure and record the patient’s pulse, respirations, and blood
pressure immediately before biopsy.
4. Describe to the patient in advance: steps of the procedure; sensations
expected; after-effects anticipated; restrictions of activity and
monitoring procedures to follow.

1.Support the patient during the procedure.
2. Expose the right side of the patient’s upper abdomen (right
hypochondriac).
3. Instruct the patient to inhale and exhale deeply several times, finally
to exhale, and to hold breath at the end of expiration. The physician
promptly introduces the biopsy needle by way of the transthoracic
(intercostal) or transabdominal (subcostal) route, penetrates the liver,
aspirates, and withdraws.
4. Instruct the patient to resume breathing.

1. Immediately after the biopsy, assist the patient to turn on to the right
side; place a pillow under the costal margin, and caution the patient to
remain in this position, recumbent and immobile, for several hours.
Instruct the patient to avoid coughing or straining.
2.Measure and record the patient’s pulse, respiratory rate, and blood
pressure at 10- to 15-minute intervals for the first hour, then every 30
minutes for the next 1 to 2 hours or until the patient’s condition
stabilizes.
3.If the patient is discharged after the procedure, instruct the patient to
avoid heavy lifting and strenuous activity for 1 week.
