Greetings and Salutations!

We are third year students pursuing Bachelor in Elementary Education at Cebu


Technological University- Malabuyoc Extension. We are writing a thesis entitled,
Parental Involvement and Classroom Behavior of Elementary pupils.

In respect, we would like to seek approval from your good office to allow us to
administer our survey questionnaire by letting Grade 4, 5, and 6 pupils to answer said
instrument. Their responses would be a great help in formulating an action plan, which
is an output of the study. The test administration will be given on April, 2023.
