1.0 INTRODUCTION
Disaster could happen at any time and at any place. There are two main sources of
disaster i.e. Natural phenomena (flood, landslides, tropical storm, lightning and etc.) and
disastrous event occasioned by man or by man’s impact upon the environment, industrial
accident, fire, fuel tanks explosion, structure collapse, electrocute chemical or hazardous
substances / material spills or leakage. Catastrophic situation of this nature can lead to
emergency situation. Therefore, the purpose of an emergency response plan is crucial for
the need of:
 To prepare people and equipment and develop procedure to minimize the

impact of an emergency.
 To prevent and mitigate the health & safety risks in the event of an incident or
emergency situation.
 To prevent and mitigate the environmental pollutions in the event of an incident
or emergency.
2.0 DEFINITION / TERMINOLOGY
a) An Emergency
Is a situation due to an actual or imminent occurrence of as the above mentioned of
which endanger, or threatens to endanger the safety of persons or destroys or
damage or threatens to property.
b) Sources
Potential risk can have its origin in natural phenomena or human activities.
c) Risks
Potential risks of different kinds may be associated with the properties of a given
source.
d) Triggering causes
Physical circumstances and / or human acts that trigger a potential risk into an active
risk thus giving rise to an accident.
e) Accident
An active risk corresponds to the event.
3.0 OBJECTIVE
The development of this ERP is to help reduce the adverse effects of any disaster by
keeping the Project Management Team prepared with necessary plan and procedures to
manage a particular emergency situation. It is therefore vital for the Team and all workers
to be well acquainted with the procedures.
4.0 SCOPE

This procedure covers every individual, equipment, tools and premises. It is oriented
towards minimizing initial impacts to the environment, employees, the community in the
neighborhood and processes in the organization. It is envisaged that emergency situation
that are likely to occur at the project site will be in the form of life-threatening injury due
to accidents, fire and spillage of chemical/ oils that may affect the environment.
5.0 HIERARCHY OF COMMAND
For ease of control during an emergency situation a predetermined chain of command for
the project is as follows:
 HSL(HQ) & Project Site (PM/CM)

 Ensure that the site is prepared for an emergency such as outbreak,
serious incident or spillage of chemical.
 Ensure that resources are available for the provision of emergency
equipment and appliances such as firefighting appliances, fire detection
system (office), first aid kits, etc.
 Ensure that resources are available for the necessary training of
Emergency Response Team.
 Overall responsible for the coordination of emergency situation and
effective Emergency Response Plan.
 Emergency Response Team

 Ensure that systems provided for emergency situation are being
properly inspected and maintained in good working condition.
 Ensure participation in Emergency Response Planning and Training to
maintain effective level of competence in attending to emergency
situation.
 Assist Incident Commander for coordination of fire and incident
emergency situation.
 Other Employees
 Understand the emergency evacuation procedures and routes.
 Strictly adhere to instructions in the emergency plan ERP.
6.0 PROCEDURES
6.1 The Emergency Response Team (ERT)
The ERT shall comprise of employees from various departments, who have knowledge
and have been trained in responding to emergencies. Each team must have a team
leader. The team shall also refer to the Project Safety Plan and Environmental
Management Plan / Environmental Management Compliance Plan for more specific
response.
Refer to Appendix 1 for the organization chart of the ERT.
6.1.1 Appointment of ERT Member

Incident Commander shall receive an official appointment letter from respective Project
Director.
Upon recommendation from the respective Department, the Incident Officer shall
propose the selected employees to the Deputy Project Director / Project Director for
approval. Each ERT member shall receive an official appointment letter from the Deputy
Project Director / Project Director, which sets out his / her new responsibilities.
6.1.2 Training
The ERT team, shall be given training relevant to their new responsibilities. Example: -

 Incident control  Emergency management designation
 Monitoring equipment  Emergency Evacuation Map

Besides the ERT members, all employees shall also be given health & safety awareness,
environmental awareness and emergency response training. The security guard must be
given proper briefing on the emergency response by the Incident Commander/Safety &
Health Officer.
6.1.3 Communication
Refer to Appendix 1 for the contact numbers of the ERT (during work hours) of the
relevant personnel (during off-work hours) and the external rescue team e.g. Fire &
Safety Rescue, ambulance, etc.
6.1.4 ERT Drills
The ERT members will go through drills (e.g. fire drill, chemical spillage drill & first
Aid drill) to practice the skills they acquired during training. The drills shall be
conducted at least once a year and alert the Fire & Safety Rescue authority on the
fire drill if required. The ERT Committee shall be responsible for coordinating all the
relevant activities. Refer to Appendix 2 for Safety Drills & Critique.
6.1.5 Availability of ERT
The ERT will be available during working hours. Should there be any emergency
after working hours, contact the Incident Commander / Safety Officer.
6.1.6 Emergency Command Centre
Where available, the guard house will be the Emergency Command Centre.
6.1.7 Medical Examination

Medical examination shall be required if any of the ERT members was found to be
incapable of performing function due to health reason / physical limitations. The
company doctor should verify and confirm if the ERT members are fit to
participate in the ERT activities.
6.1.8 Legal Reporting
Where required by laws, the safety & health officer shall report health & safety incidents
within the legal time limit after consultation with the Incident Commander.
6.1.9 Self-Assessment Checklist
During the Safety & Health Review, the Incident Officer shall administer a self-assessment
checklist (Appendix 3) to review the company's emergency preparedness and response.
6.2 Activities during Fire Emergency / Explosion
6.2.1 Emergency Response - Fire Emergency / Explosion
The Safety & Health Officers and the Incident Commander who have the knowledge
of the site conditions and local regulations shall prepare the Emergency Response
Plan (ERP) for fire / explosion, taking into consideration the financial and human
resource availability. The Emergency Response Team (ERT) may be involved in
the preparation of the plan or contribute to the continual improvement of the plan
from time to time.
6.2.2 Notification
The first individual aware of the emergency occurrence shall inform the Safety Personal /
ERT Leader / any of the HSE Committee member. The Safety Personal / ERT Leader /
HSE committee member shall then notify and request the relevant ERT members to
assemble at the emergency site. The notification for the assembly will be initiated
by the emergency alarm.
6.2.3 Fire Emergency Signal
Fire Emergency Signal shall be determined and rectified upon practicability as per
usage according to the work and site location and condition.
6.2.4 Shutdown of Certain Utilities and Services/ Operations
During an emergency (fire / explosion), it may be necessary to shutdown gas,

electricity water and other services / operation.
 Machine operator is to shut down the operation by activating the emergency
stop,
 All maintenance jobs shall be stopped immediately.
 Flammable material such as thinner shall be placed in safe place as far as

possible.
 Flammable material shall not be taken out during evacuation.
6.2.5 Fire control
The person that spot the fire shall try his best to fight the fire immediately with fire-
Extinguisher, if it is not endangering to his / her life. And when it is beyond the
control call for the ERT (fire-fighting team) for assistance.
The respective section leader shall ensure that all employees in the section are
evacuated from the affected areas if there is an immediate threat to human life. During
evacuation, everyone shall move in an orderly manner through the designated routes
identified in the evacuation map. Refer to Appendix V.
6.2.7 Accounting for Employees at the Assembly Points
It is the respective section leader's responsibility to account for all the employees at the
assembly points from the specified section and immediately notify the assembly leader of
any missing employees. Employees should be forewarned not to re-enter the area until all
clear signals are given by the ERT Leader.
6.2.8 Assessment of the Emergency
The Fire-Fighting Team members shall inspect the emergency area to ensure that all
employees are away from the area and make an assessment of the emergency situation.
6.2.9 Evacuation Maps & Assembly Points
Up-to date evacuation map shall be displayed in numerous locations in the

organization / site. This map shall show the closest exit, backup exit and the
location of assembly points. All employees shall know the primary exit route in case of
fire/ explosion.
6.2.10 Remove Injured Employees
If injured employees are found, the ERT Team members who are equipped with PPE
should carefully remove the injured out of the emergency area, first aid shall be given
while waiting for the ambulance's arrival.
6.2.11 Call to Outside Resources / Agencies
If immediate assistance is needed, any external or internal personnel may call the fire
department, ambulance and other type of services.
6.2.12 Evacuation Practice for Fire/ Explosion Emergency.

At least once a year, all employees will undergo an evacuation drill. If the drill disrupts
the work schedule, then each department shall carry out separate drill. Based upon notes
taken during the drill, a report shall be given to all Department Heads and Section Heads
concerning the outcome of the drill. Any special problem encountered should be
investigated through incident report.
The ERP shall be reviewed at least once a year or when significant changes have
occurred.
The ERP will be amended when important components become outdated or business and
regulatory changes occur. Whenever changes are made, a revision date and appendix
number shall be noted. The updated revision will be issued to all relevant personnel.
At least one copy should be in every building including the guardhouse. In addition, the
following individuals should have their own "Controlled copy".
 Emergency Response Team leaders (firefighting team, first aid team and etc.)
6.3 Activities during Flood at construction site
6.3.1 Preliminary preparedness of flood
Site SHO shall plan the emergency activities for ongoing / continual raining seasons in
construction site, especially underground works. High possibility of flood may occur in
raining season and an Emergency Plan for flood shall be prepared.
6.3.2 Training / Tool box
Meeting shall be conducted to brief employees, sub-contractors or other interested

parties for activities to be done during flood case.
6.3.3 Notification
The first individual aware of the emergency occurrence shall inform the ERT Leader / any of
the OSH Committee member. The ERT Leader/ OSH committee member shall then notify
and request the relevant ERT members to assemble at the emergency site. The
notification for the assembly will be initiated by the emergency alarm.
6.3.4 Shutdown of Certain Utilities and Services/ Operations
During an emergency (flood), if it may be necessary to shutdown gas, electricity, water

and other services.
 Machine operator is to shut down the operation by activate the emergency stop.
 All maintenance jobs stopped immediately.
 Flammable material such as thinner shall be closed all the time to prevent others emergency

6.3.5 Subsequent flood control activities shall follow clause 6.2.6 - 6.2.13.
6.4 Activities to prevent outbreak of disease
 All subject is capable of transmitting a variety of diseases to human, some more

than others, consequently, physical contact with the subject must be avoided
and strict personal hygiene rules must be observed.
 Staff who regularly handle subject should seek the advice of their physician for
appropriate immunization. The following are more specific health and hygiene
measures to be adopted for the protection of personnel whose work brings them
into contact with subject.
 Handlers of subject must wear face mask and gloves whenever possible and
the containers of such subject must carry a warning to these effects or
alternatively, cover the ventilation opening of the container with light
weight material that does not include ventilation, to prevent possible
inhaling of infectious by handlers.
 Report to a doctor as soon as possible after being contact by subject providing
information on the origin of the subject. If delay is inevitable before obtaining
medical attention, ensure that the wound is thoroughly washed with soap and
water and covered with a dry dressing until medical attention can be obtained.
 Avoid contaminating skin or clothing with blood or excreta of subject.
Contaminating clothing must be changed and send for cleaning. Skin
contamination must be cleansed.
 Staff who are frequently involve with subject handling must be immunized against
those disease which are transmissible to human.
 Suspected sick subject must not be quarantine with other subject at all times
because of serious risk of contamination.
7.0 DUTIES AND RESPONSIBILITIES
7.1 Incident Commander – in the event of emergency
i) To determine the location of Emergency Control Center.
ii) To issue directive of emergency and evacuation.
iii) Recommend with appropriate course of action to overcome and manage emergency
situation.
iv) Directing all resources in the affected area including evacuation of personnel and
minimizing loss of property.
v) Ensuring that external emergency services such as Fire and Rescue Department
(Bomba) or Medical Aids are notified, when necessary.
vi) Issue of project operation shutdown, when necessary.

vii) Notifying of Project Emergency Response Team’s withdrawal if the situation is beyond
control and the handover of emergency situation to relevant authority, if requested or
necessary.
viii) To provide detail report of emergency status inclusive of highly hazardous area,
materials or substances available in the project worksite to the relevant authority, if
requested or necessary.
ix) To declare the termination or cessation of emergency and evacuation period.
 To issue directive for mobilization of project clearing up team, when necessary.
 To review recovery arrangement inclusive of emergency event impact evaluation

and reviewing of the Project Emergency Response Plan.
 To arrange and ensure adequate resources for the development of Project

Emergency Response Teams.
 Issuing directive for conducting Emergency Evacuation mobilization drill for the
project.
 Issuing directive for the inspection of the project emergency equipment by the
relative emergency coordinators.
7.2 Historian
i) The primary duties and responsibilities of Secretary is to record all sequence

of incident activities. Update the Incident Commander on the records and
statistics of the incident.
ii) Other than that, Historian to assist in all key level of Emergency Response
Team.
7.3 Damage Control Coordinator
The primary duties are as below and not limited to:
i) To render assistance to Emergency Controller, Deputy Emergency Controller

and any other members of Project Emergency Team in handling emergency
situation.
ii) To organize and coordinate the establishment of Project Fire Fighting Team.
iii) To organize and coordinate Fire Fighting Exercise / training and inspection of
Fire Fighting Equipment periodically during non-emergency situation.
iv) To coordinate and assist Fire and Rescue Services Department (BOMBA) in the
event of emergency.
7.4 Rescue Coordinator

i) To render assistance to Emergency Controller, Deputy Emergency Controller and

any other members of Project Emergency Team in handling emergency.
ii) To coordinate and organize rescue operation within own organization or

external organization.
iii) To carry out rescue plan issued by Emergency Controller or Deputy Emergency
Controller in the event of emergency.
iv) To furnish Emergency Controller or Deputy Emergency Controller with the

report of rescue operation status.
7.5 First Aid Coordinator
i) To render assistance to Emergency Controller, Deputy Emergency Controller

and any other members of Project Emergency Team in handling the event of
emergency.
ii) To organize and coordinate the setting up of temporary first aid center in an
appropriate location during the event of emergency.
iii) To organize and coordinate training for the project first aid team.
iv) To render full co-operation and assistance to external first aid agency during
the event of emergency.
7.6 Communication Coordinator
The primary duties are as below and not limited are:
i) To render assistance to Emergency Controller. Deputy Emergency Controller

and any other members of Project Emergency Team in the event of emergency.
ii) To organize and coordinate the establishment of Project Communication Team.
iii) To organize and coordinate the setting up of communication network between

Emergency Command Center with:
 All other Project Emergency Response Team.

 The nearest Fire and Rescue Services Department (Fire Brigade).
 The nearest Hospital/ Clinic.
 The nearest Police Station.
 HSL – Head Quarters.
 Sarawak Energy Berhad.

 Local DOSH / JKKP.
iv) To organize for runners or drivers in case it is required, to substitute other

means of communication which may not be possible.
7.7 Evacuation Coordinator
The primary duties are as below and not limited are:
i) To render assistance to Emergency Controller, Deputy Emergency Controller and

any other members of Project Emergency Organization in handling the event of
emergency.
ii) To organize and coordinate the Project Evacuation Team.
iii) To render full co-operation and assistance to external Evacuation Agency/ Team
during the event of emergency.
iv) To organize and coordinate training for Project Evacuation Team.
v) To develop procedure/ guideline for Project Evacuation Team.
vi) To coordinate and provide the following data/ status to Emergency Controller in
the event of emergency particularly pertaining to evacuation operation:
 Number of personnel being evacuated.

 Number of discrepancy or number of missing personnel.
 Status of major equipment.
 Number of rescued casualties.
 Any other information that is necessary for Emergency Controller and Deputy
Emergency Controller.
 Report or headcount status.
7.8 Traffic Control Coordinator
i) To render assistance to Emergency Controller Deputy, Emergency Controller and

any other members of Project Emergency Team in handling the event of
emergency.
ii) To organize and coordinate the Project Traffic Control establishment.
iii) To organize and ensure the availability of access for emergency rescue vehicle.
iv) To render full co-operation and assistance to external supporting agency in the
event of emergency.

v) To coordinate and develop guidelines/ procedures for Traffic Management
Control in the event of emergency.
8.0 CONCLUSION
Disasters or accidents are consequences that call for immediate action, whether at the site
level or project level. Therefore, preparing the total work force with appropriate written
guidelines, procedures and training is essential in minimizing the impact of disasters or
accidents.
Therefore, it is the duty of every front-line Management Staff of Hock Seng Lee Berhad to
promote and communicate the content of this plan to their subordinates. The total
understanding of this plan will assist us in handling emergency situations in a more
prepared and organized manner.
