CARD-MRI Development Institute, Inc. (CMDI), a private educational institution duly organized and existing under
the laws of the Republic of the Philippines, with principal place of business at Purok 3, Brgy. Tranca Bay, Bay, Laguna,
hereby grants the College Scholarship to Nolasco, Adriel Jezreel Jimenez with Student Number 01-03-21-0560 a
resident of 478 Sitio Ibaba Brgy. Sto Domingo Bay, Laguna under the following terms and conditions:
A. Nature of Scholarship
1. The scholarship is granted on the basis of status as deserving student who belong to socio-
economically challenged family eager to finish studies at CMDI;
2. The scholarship is granted based on one CMDI scholar per household. Extending number of
qualified students to up to two scholarship slots per household may be allowed subject to further
assessment and approval of CMDI. This is, however, not automatic but depends on management
discretion based on reasonability, prioritization on most in need and funds availability;
3. The scholarship covers a full amount of school fees except the graduation fees, books, and other
expenses.
4. The Scholarship is granted up to (4) years completion of the course.
5. School fee is directly paid to CMDI;
6. The scholarship may be granted or availed every first semester of the year;
7. In cases where the grantee fails to finish his/her degree, dismissed or transferred to another
school, except due to prolonged illness, accident or death of a parent or guardian, the grantee
shall repay the amount of school fees to cover the previous semester and current semester (if
on-going), he/she is enrolled. In addition, shifting fees will also be shouldered by the grantee
should he/she decide to shift to another programs. If monies are not paid, CMDI may seek
reimbursement through legal action; and
8. CMDI reserves the right to modify or amend the terms and conditions of this agreement as
deemed necessary due to any violation of the grantee and/or other reasons beyond the control
of the concerned parties.
B. Academic Requirements
1. A student may still qualify for scholarship even if he/she has taken other programs outside CMDI
and he/she wishes to continue at CMDI, either he/she will take the same program or shift
program and have his/her completed course be applied for crediting in the program he/she will
take at CMDI;
2. The students are highly encouraged to have an excellent academic performance in line with our
goal of completing the academic program by having a quality education as believed that
education is a key tool for poverty eradication;
3. The scholar must pass all subject/course taken every semester;
4. The scholar may drop a subject/course, but any cost including unrecoverable tuition paid for the
subject/course will be deducted from the total amount of scholarship grant for the succeeding
semester;
5. The scholar must complete the requirements of a subject where he/she got an “incomplete”
grade immediately upon offering of the subject/course;
6. The scholar must finish within the normal duration of the course or curricular program, any
extension beyond the period of scholarship granted will not be covered; and

7. Courses taken within the regular semester or summer/special term which are not mandated or
part of the curricular program of the degree are not covered in the scholarship grant and will be
paid by the student.
In addition, shifting fees will also be shouldered by the grantee should he/she decide to shift to another programs. If
monies are not paid, CMDI may seek reimbursement through legal action; and
Based on the condition above, the amended scholarship grant is given for 4 years effective First Semester, A.Y. 2021-
2022 until Second Semester, A.Y. 2024- 2025.
By indicating my/our conformity herein, I/We agree with all the terms and conditions in this contract.
