APPLICATION FOR ENTRY OF A PRIVATE CAVEAT
To the *Registrar/Land Administrator Selangor
I, CHONG CHEE KEONG (NRIC: 590102-10-4567) of No. 32 Taman Sentosa 41200,

Klang Selangor Darul Ehsan hereby apply for the entry of a caveat upon the title to the land
described in the schedule below; to be expressed to bind—
*the land itself/the particular interest described in the said schedule.
2. The grounds of my claim to the *land/interest are—
(a) Cheong Kok Yong ((NRIC: 540512-42-5405) has purchased the land GM 520, Lot
7044, Mukim Tanjung Dua Belas, Daerah Kuala Langat, Negeri Selangor Darul Ehsan
(hereinafter referred to as “the Land”) and the purchase price was paid by Yew Lian
Door Frame, a business owned by CKY and his three brothers.
(b) Yew Lian Door Frame then changed its name to Sykt Union Wood Industries (SUWI)
which was later incorporated using the name Sykt Union Wood Industries Sdn Bhd
(hereinafter referred to as “the Company”).
(c) The company has taken over all assets and liabilities of SUWI including the Land.
The Land is registered under the names of individual persons, Chong Ket Kin (NRIC:
580118-10-5801), Chang Kok Soi (NRIC: 570229-10-5702), Cheong Kok Yong @ Ku Lon
(NRIC: 540512-42-5405), and Chong Yoon Min (NRIC:550623-42-5506) (hereinafter
referred as the “Registered Proprietors”) with each of the Registered Proprietor are
holding ¼ share of the Land as the trustees for the company during their lifetime.
(d) Cheong Kok Yong and the administrators of the three Registered Proprietors have
brought several suits and originating summons regarding the Land since 1997. I have
entered this Private Caveat on the property, to protect the land and its interest as Sykt
Union Wood Sdn Bhd is holding the land on trust.
3. As required by section 323 of the National Land Code, I now submit—
(a) the prescribed fee

the prescribed fees of RM 300.00;
(b) a statutory declaration of *myself/my advocate and solicitor verifying the claim set
out in paragraph 2 above; and
(c) if relating to a part of the land, a description or a plan of the land affected, or if
relating to an undivided share in the land or part thereof, a description thereof.
**4. The last known address of the proprietor in my knowledge is:
