    (Exclusive for the parents of those pupils who will allow their child to attend
    the face-to-face classes)
    April 2, 2022 (Saturday)
    9:00 am – 10:00 am
    Agenda:

     Limited Face-to-face School Implementation


    - Dry Run Schedule for the face-to-face classes
    - Signed Parent’s Consent and Waiver Form
    - Limited Face-to-face Class Schedule
    - Weekly and Daily Health Declaration Forms
     Signages to be followed
     First Communion
     Financial Report
     Gulayan sa Paaralan
     Other matters
    - Showing of Cards

    I. Roll call and Determination of Quorum.


    The meeting started with a short prayer thru power point
    presentation. It was presided by the class adviser and the HRPTA Vice
    president made the minutes of the meeting and took some
    documentation.

    II. Call to order


    The meeting started at 9:10 am. There are 24 parent participants in
    the meeting.

    III. Adoption of the Agenda.


    All agenda was delivered clearly, and all the parents agreed and move
    to absorb those agenda.

    IV. Presentation
    Parent’s meeting of Grade IV- TANDANG SORA Class was presided by
    the class adviser. She tackled about the agenda mentioned above and
    added some reminders on the obligation of parents as they will send
    their child in school during the implementation of the Dry run and
    the actual face-to-face classes. There are some questions asked
    during the open forum and she successfully answered them all. After
    the meeting, she facilitated the showing of report cards.
