    AFFIDAVIT

    I, AMELIA M. PABRUA, of legal age, Filipino and with residential and post office
    address at 280 Garnet St. Adelina 1A San Antonio, City of San Pedro, Laguna, Philippines, after
    having been duly sworn to in accordance with law, hereby depose and say;

    1. That, I am Certified Public Accountant licensed by the Professional Regulation


    Commission with Registration No. 0055678-97;

    2. That, since September 2003 until the present time, I was not able to practice my
    profession instead being a full time housewife;

    3. That, verified by this affidavit, to be a matter of record, now and for all times, that I did
    not practice my profession from September 2003 until present;

    4. That I am executing this affidavit in order to attest to the truth of the foregoing
    statements, to inform the authorities concerned, for filing of retirement in SSS and for all
    legal intents and purposes this may serve best;

    IN WITNESS WHEREOF, I have hereunto set my hand and affix my signature


    below, this 23rd day of September, 2015 at the City of San Pedro Laguna.
