    SWORN AFFIDAVIT

    That I, ANGELA RAYMOND FERRERIA TAN, married, and of legal age, Filipino, with
    office address at Unit 1803 Atlanta Centre, 31 Annapolis St., Greenhills, San Juan, after
    having been duly sworn to in accordance with law, do hereby depose and say that:

    1. That I am the Managing Partner of BARISTA AND COFFEE ACADEMY OF ASIA


    INC. located at the above-mentioned office address;

    2. That on June 21, 2019, we request for a Certificate of No Pending Case with DOLE-
    NCR as requirements for bidding;

    3. That however, we are still waiting for the response from DOLE-NCR, attached
    herein is the Request Letter received by DOLE-NCR as part of and marked as “ANNEX A”;

    4. That as such there is no pending administrative / criminal investigation against


    BARISTA AND COFFEE ACADEMY OF ASIA INC.;

    6. That I am further executing this affidavit to affirm the truth of my foregoing


    statements and for whatever legal purpose it may serve me best.
