Screening Process
OCHN’s professional access and referral team members work with individuals and families to
identify their needs, and schedule an appointment if necessary.
What To Expect
Licensed clinicians ask questions about mental health, intellectual/developmental disabilities
and/or family history, risk of harm, physical health needs, medications, functional status, family
relationships and stress, substance use, socialization, school, symptoms, etc. Substance Use
Disorder (SUD) screenings are also conducted prior to a person being discharged from a
hospital to ensure appropriate follow-up care is in place.
Eligibility Determination
Individuals who qualify for services are given a choice of providers and linked with them.
Appointments occur no more than 14 days for adults, seven (7) days for children, after a
screening determination is made.
OCHN does not exclude people or treat them differently because of race, color, national origin, age,
disability, or sex. OCHN is funded, in part, by the Michigan Department of Health and Human Services.
