    I, DENNIS BASILIO GARCIA, of legal age, Filipino, and presently residing at


    192 SD, Brgy. Pasong Camachile II, General Trias, Cavite, after having been duly sworn
    to in accordance with law, do hereby depose and say:

    1. That I am the owner of the following certificates:

    a. Notarized copy of Confirmation of Employment issued by Tetra Pak


    Philippines Inc. A machine copy of my notarized Confirmation of
    Employment is hereto attached as Annex “A”;

    b. Notarized copy of Diploma issued by Bulacan State University. A


    machine copy of my Notarized Diploma is hereto attached as Annex
    “B”;

    c. Notarized copy of Transcript of Records (TOR) issued by Bulacan State


    University. A machine copy of my notarized Transcript of Records
    (TOR) is hereto attached as Annex “C”;

    2. That I am executing this affidavit to attest to the genuineness and authenticity


    of the abovementioned Certificates for my Philippine Overseas Employment
    Administration (POE) Compliance Application and for whatever legal
    purpose it may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 17 th day of November


    2022, in Baliwag, Bulacan.
