Board Resolution for the Purpose of Accreditation
Know men by these presents;
We, the concerned and responsible among members and the community, in order to establish our
organization shall everybody shared values and ideas, protect our recent and uphold our dignity
as human being. We promulgate harmony and productivity and progress among ourselves and
goodwill.
Whereas San Rafael Cops (Civic Organization fo Prevention and Suppression of Crimes)

The organization Lambat Sibat Program duly organized and existing under Philippines laws.
Duly Approval Board Resolution signifying intension for Accreditation and for the purpose of
representation in the local special body.
Whereas San Rafael Cops (Civic Organization for Prevention and Suppression of Crimes) do
hereby ordain promulgate this resolution as constitution and bylaws.

The organization exercise the peace in order provides sustained humanitarians, unity productivity
and volunteerism.
The organization exercise the peace in order, leadership, volunteerism in collaboration with the
Municipal and provincial government in Bulacan.
Now therefore, premises considered the board hereby approved and the following.
Duly Approval Board Resolution signifying intension for Accreditation and for the purpose of
representation in the local special body
Resolve further: that the board authorize the President and Board Secretary to act as its
representative to transact application for accreditation.

We as a civil society organization / a civic organization for prevention and suppression of crimes
our mission and vision recognized the office of Brgy. Hall of Mabalas – balas.
We truly accomplished these requirements for accreditation of Sangguniang Bayan and who is a
volunteer group of DILG.
We are also member of large organization in Bulacan (BCCSO) Bulacan Coalition of Civil
Society Organization for postering unity and camaraderie were organized the good leadership
with corresponding programs in collaboration with the provincial government of Bulacan.
After sharing values and contributions, followed of participationg clean up drive and rendering
services for peace in order. We were having a preparation for first aid rescue training in
coordination of Red Cross. As an organized group.

Annual Accomplishment Report
2021
Being Civic Organization we uphold our dignity as human being and rendered our voluntary
services.
We support the Bulacan Coalition of civil Society Organization (BCCSO) the Fun Run last July
14, 2017 at Baliwag Bulacan , for “Takbo pa ra sa biktima ng kalamidad” with benefits.
We also join the “Fun Run” of Municipality of San Rafael with our Mun. Mayor Cipriano D.
Violgao Jr. for Peace in order (Kontra Droga)
SR COPS, have a courtesy call to the newly elected Brgy. Captain and Brgy. Officials of
Mabalas – balas, Hon. Jennifer Vasallo, We Discuss to them the objective the mission and vision
of the organization.
As of July 2018 we seek a relief goods from Brgy. Officials and Municipal Officials for
Hagonoy who suffering “Baha”.
The Maguinao Elementary School, headed principal Anne P. Castillo have the biggest
contribution among from their students. First batch of our relief goods distributions in the Brgy.
Sto Rosario Alanganin of Hagonoy Bulacan. the 2nd Batch is in San Miguel Hagonoy.
As a volunteerism we are 24/7 willing to serve in our community so:
When Vixen Enterprises held their Christmas Gift Giving then invites the SR COPS to join and
participate for the distribution of the gifts to the children they invited to their Brgy.
To promote our organization so the willness of the public services, cooperation to the community
programs and volunteerism and to become member of the large organization of Bulacan, we
poster our camaraderie and goodwill.
