I. NATURE

The DOST-SEI _______ Junior Level Science Scholarship is awarded to a deserving student in his/her
third year of college to enable him/her to pursue higher education or training in the areas of science, technology,
engineering and mathematics.

II. DURATION OF THE SCHOLARSHIP

The Party of the Second Part shall be entitled to an award for the last two (2) years of his/her undergraduate
study if he/she is enrolled in a four (4) year program; or the last three (3) years if he/she is enrolled in a five (5)-

year program, unless earlier terminated due to his/her failure to comply with the terms and conditions of this
Scholarship Agreement.

III. STUDY PLACEMENT

The Party of the Second Part shall continue to enrol in the DOST-SEI approved institution or
college/university with CHED-compliant program/s where he/she is originally admitted at the time this scholarship is
awarded. He/she is not allowed to transfer to another university or shift to another program.

IV. ENJOYMENT OF ANY OTHER SCHOLARSHIP AWARD

The Party of the Second Part may be allowed to enjoy a supplementary scholarship administered by the
university/college where he/she is enrolled in, or to avail of financial assistance from another government agency or
local government unit as supplementary grant for tuition and other school fees only.

The Party of the Second Part may be allowed to enjoy a private scholarship administered by his/her institution
simultaneously with the DOST-SEI Scholarship, provided that the terms of the private scholarship do not interfere
with his/her contractual obligation with DOST-SEI. Availment of the private scholarship must be approved by DOST-
SEI.

If the Party of the Second Part decides, during the period of his/her DOST-SEI Scholarship, to transfer to a
private scholarship, he/she shall be required to render service in the Philippines after he/she graduates, on full-time
basis, preferably in his/her home region, for a minimum period equivalent to the length of time that he/she enjoyed
the scholarship.

V. SCHOLARSHIP PRIVILEGES

The Party of the Second Part in good academic standing shall be entitled to financial assistance consisting
of school fees, book allowance, transportation expenses, monthly living allowance, group health and accident
insurance and graduation allowance during the period of actual study to be disbursed by the Party of the First Part,
subject to the availability of funds for the purpose and to pertinent government accounting and auditing rules, as

follows:

A. During the regular academic year

1. School Fees – Tuition and other school fees based on Statement of Account/Bill/Official Receipt but
not to exceed P40,000.00 per academic year;
2. Book Allowance – For prescribed textbooks and essential school supplies to be released as outright
grant at P10,000.00 per academic year;
3. Transportation Expenses – One actual economy class round trip per academic year from the
permanent residence of the scholars to the place of study outside their home province/region.
(Reimbursement shall be made at the start and end of academic year only).
4. Monthly Living Allowance – For the ___-month duration of each academic year at P7,000.00 per

5. Group Health and Accident Insurance
6. Thesis Allowance – P10,000.00, outright grant after submission of the signed approval sheet of the

thesis proposal and one-page abstract; and


7. Graduation Allowance – To be released as outright grant during the last semester or term of the
program of study.

B. During the summer term if required in the curriculum

1. Tuition and other school fees not to exceed P1,500.00;


2. Book allowance not to exceed P500.00, actual purchase supported by official receipt; and

3. Living allowance for two (2) months.

VI. SCHOLARSHIP OBLIGATIONS


Scholar

A. Grade Requirement

The Party of the Second Part shall maintain a passing grade in all academic subjects every
semester/term until completion of the course.

Should the scholar incur any academic deficiency, his/her academic performance shall be evaluated
based on a guidelines approved by the Advisory Committee on Science and Technology Scholarships (ACSTS)
or shall be elevated to the ACSTS, through the National Selection and Technical Committee, for deliberation.

B. Academic Load

The Party of the Second Part shall enrol the required regular academic load for each school
semester/term.

C. Reports

The Party of the Second Part shall submit the following reports, duly certified, on the indicated period:

1. Periodic Reports: Copy of registration form or certificate of enrolment at the start of the semester
and report of grades for every academic subject taken at the end of each semester.

2. Special Reports: Reports on curriculum or program of study; bank account number; practical training;

approved thesis proposal and one page thesis abstract (title, problem, rationale/objectives and
methodology); problems encountered; travels made; changes, if any in residence address, civil status,
and legal guardian; comments which may be relevant to his/her study program; and such reports that
may be required from time to time.

3. Final Report: Copy of Transcript of Records (TOR) on completion of the course and on immediate and
future plans for employment/professional advancement, and other requirements relevant to the DOST-
SEI Junior Level Science Scholarship Program within one (1) month after graduation.

4. Yearly Report: Report on employment status and professional advancement not later than December
30 of each calendar year for 2 or 3 consecutive years after graduation.

D. Practical Training

The Party of the Second Part shall undergo practical training in any of the laboratories of the DOST
and its agencies or at any of the research institutions/private company during the summer immediately
preceding his/her last year in college. Monthly stipend will be provided for the duration, subject to the
availability of funds for the purpose, number of hours completed and to pertinent government accounting
and auditing rules.

E. Leave of Absence

The Party of the Second Part may be allowed an official leave of absence subject only to the
following conditions:

1. The leave is highly justified, supported by pertinent documents;


2. A written permission from the university is granted;
3. The maximum duration of the leave is one (1) academic year and the expiration date shall be immediately
before the start of the ensuing semester; and

4. A report is submitted immediately upon his/her return to college. It will be used as basis for determining
the validity of the leave and the resumption of the scholarship privileges.

F. Physical Status and Moral Character

The Party of the Second Part shall maintain good physical condition and moral standing.

VII. SUPERVISION

The Party of the Second Part shall be under the supervision and guidance of the Party of the First Part and
shall comply with all scholarship policies including those not indicated in this Agreement.

He/She is not authorized to close his/her bank account unless he/she has secured clearance from the Party
of the First Part.

The Party of the First Part shall submit his/her name to partner government agencies such as the NBI and
other government agencies for tracking purposes.

VIII. TERMINATION OF SCHOLARSHIP

The Party of the First Part reserves the right to terminate this scholarship for failure of the Party of the

Second Part to maintain the grade requirement; failure to submit the periodic reports for two (2) consecutives
semesters; or to comply with any of the other terms and conditions of this Agreement effective as of the date

indicated in a written notice to that effect to the Party of the Second Part.

If the scholarship of the Party of the Second Part is terminated for failure to meet the academic requirement/s
he/she upon completion of the program, shall either render service in the Philippines as stipulated in Section IX for
a period equivalent to the length of time that he/she enjoyed the scholarship or refund the total financial assistance
he/she received with 12% interest.

IX. SERVICE IN THE PHILIPPINES

A. RA 7687 / RA 2067 (Merit)

The Party of the Second Part, immediately upon completion of his/her Bachelor of Science degree
in the priority S&T program, must render service in the Philippines, preferably along his/her field of
specialization, for a minimum period equivalent to the length of time he/she enjoyed the scholarship.

The Party of the Second Part, immediately upon completion of his/her Bachelor of Science degree
in the priority S&T program, must teach full-time high school subject(s) in Mathematics, Biology, Chemistry,
or Physics, or Information Technology or Agricultural, Aquaculture or Fisheries Technology, both in the
university and technical-vocational tracks in the secondary education curriculum in the Philippines, for a
minimum of two or three (2 or 3) years, in a public or private high school in the country.

X. REFUND OF SCHOLARSHIP GRANT

The Party of the Second Part shall refund the total amount received in connection with this scholarship for
any of the following causes:

❖ Commit breach of contract;


❖ Withdraw from the scholarship, with or without permission;
❖ Commit gross misconduct; or
❖ Fail to render service as stipulated in Paragraphs VIII and IX of this Agreement.

Refund shall be with 12% interest.

XI. TRAVEL ABROAD

The Party of the Second Part who shall travel out of the country while still under the DOST-SEI scholarship
or with service or financial obligation may be issued temporary clearance upon submission of the required documents
and/or posting a bond with the GSIS or by cash bond to be paid in instalment term not to exceed the five (5) year
maximum period.

XII. OBLIGATION OF THE PARENT/LEGAL GUARDIAN

Second Part in this Scholarship Agreement hereby acknowledges, agrees and binds himself/herself to:

1. Assume the civil liabilities that the Party of the Second Part may incur under the above written agreement as well
as civil liabilities which he/she may incur against other persons;
2. Shoulder the medical and other expenses in case of sickness, disability or death of the Party of the Second Part;
3. Provide for other expenses of the Party of the Second Part which are not specifically mentioned in this Agreement;
4. Help and cooperate in the moral, spiritual and mental development of the Party of the Second Part; and
5. Assume the obligation to pay whatever amount the Party of the Second Part shall be required to reimburse in the
event that he/she refuses to comply with the provisions of Paragraph IX or if this scholarship is terminated in
accordance with the provisions of Paragraph X above.

XIII. DATA PRIVACY

The Party of the Second Part hereby authorizes the DOST-SEI and its instrumentalities, to collect, process,
update, store, use, transfer, archive, delete his/her personal information, including those classified as sensitive
personal information, in relation to this Scholarship. The Party of the Second Part further expresses consent to submit
his/her personal data to DOST-SEI after completion of his/her program of study for the DOST-SEI Tracer Study of
Scholars and also for his/her possible employment as requested by companies.

The Party of the Second Part affirms his/her rights as data subject as provided for in Republic Act 10173,
otherwise known as the Data Privacy Act of 2012, its Implementing Rules and Regulations, and other applicable
issuances of the National Privacy Commission.
