    Dear Parents / Guardians, Date: 14/09/2021

    The school is in the process of constituting the school level fee committee (SLFC) of
    the parents whose ward(s) is / are studying in the school (PTA) for the academic
    session 2022-2023. Due to COVID-19 protocol we are choosing the online
    procedure.

    Eligibility / conditions

    1. PTA members who give the consent through this Google Form or written
    application submitted to the school office by hand dated on or before
    24-09-2021 will be considered. The written application should reach the
    school office by 10:00 a.m. on 24-09-2021, the last date for Google form
    submission is also the same (If the applicant submits hand written application
    for SLFC then all the relevant details asked in the Google form should be
    included in the letter)
    2. The ward should be studying in the school in the academic session 2021-2022
    in classes 1 to X.
    3. If more than one child is studying, only one consent form per family will be
    considered.
    4. Should have sufficient knowledge about the functioning of the school system,
    financial matters and relevant guidelines of the Government on SLFC related
    issues.
    5. Should not be a habitual defaulter in paying fees (as on 24-08-2021 in our
    school records, fees mandated by the Supreme Court verdict concerning
    academic session 2020-2021 should have been compiled with and Ist quarter
    fees of 2021-22 must have been cleared)
    6. Has been always cooperative with the school
    7. The applicant should upload his / her highest qualification degree certificate /
    mark sheet.
    8. As per the Act, the application fee of Rs. 50/- is to be paid online using the
    below given details to become a member of SLFC and the screen shot of the
    payment should to be uploaded.
