document (e.g. TOR, official grade report, date of issue, etc) used as reference of the crediting process.

TO THE TRANSFEREE: Bring this form with your TOR to the chairperson of the department in XU offering the specific subject you want to be credited. On the first 3
columns below, write the course code & number, number of units and grade obtained in another school. Do this on the specific row corresponding to the subject in XU
(as indicated on the last 2 columns). The chairperson will indicate his/her name, sign (with date) and write "CREDITED" if the subject/s is/are credited; if not, "NOT
CREDITED" will be written.

TO THE SHIFTEE OR FOR INTERNAL CREDIT: Bring this form with your TOR/official grade (i.e., SLMIS printout authenticated by the College Dean) to the different
chairpersons for crediting of subject/s taken in XU which has/have different course code/s title/s, and/or description/s in the program curriculum you are enrolling to
or as advised by the evaluator.

TO CROSS-ENROLLEE: Bring this form with your TOR reflecting/official grade of the cross-enrolled subject (i.e., taken in another school) to the chairperson in XU

IMPORTANT NOTES:
(1) 50% of total units in a program must be taken in XU in order to be given an XU diploma (Refer to Student Handbook).
(2) To be eligible for graduation honors/awards, a student must have completed at least 80% of the total academic units required for
graduation in XU (refer to Student Handbook).
(3)This form must be accomplished in THREE (3) copies. After receiving/signing by the Registrar's Office, submit 1 copy to your
Department Chairperson, 1 copy to Window 8, Registrar's Office on or before the end of the semester of entry, otherwise the student
forfiets any transfer credits from other school. Third copy should be kept by the student and should not be lost since this document
is very important to establish the completion of your academic program.
