1 Whether all site workers been gone through Tool Box talk prior to start of work?
Prior to start of excavation work, Check for any underground electrical cables,
2 OFC cable, sewer pipe, water pipe, any gas pipe…etc.
Are all workers using proper PPEs at site, i.e. Safety Helmets, Gum Boots/ safety

shoes, Rubber hand Gloves, Nose mask, etc.?

During excavation, Whether Excavator / JCB placed at a safe distance (Min 1.5m

away from edge) to avoid fall down into the pit?
Whether safe distance of dumped excavated soil of all four sides from the edge

of the pit is being maintained.(i.e. 1.5 mtr.)?

Whether angle of repose/slope of pit is as per design or requirements?

If required ,proper installation of Shoring /Shuttering has been done ?
Whether MS ladder of adequate length has been provided in the pit &
9 positioned at correct angle(70 degree)?
Is the work area suitably cordoned off? Adequate signage, posters, banners &

caution tape are present at site for public address?
Is the template for supporting the stub is placed in safe manner with jacks on

level ground? All nut bolt fixed as per drawing?
Is the prop for supporting the stub is placed in safe manner with anchor bolt?

All construction materials / Concrete Mixer / De-watering pump placed at a safe
13 distance i.e. 1.5 mtrs. from edge of pit.
Whether Concrete mixer is in good condition & all moving parts of it is properly
14 guarded & it's wheel is properly blocked to avoid any movement?
If location is water logged area, proper pumping & de-watering arrangement has
15 been done.
Is lone / single worker allowed to work in the pit?

If night work required, proper approval / permission has been given &
17 appropriate additional safety measures has been taken. Proper lighting
arrangement has been done?
For night work, all electrical connection should be proper. Cables are free from

joint / damaged?
Is adequate first-aid box available at site & emergency contact No. available with
19 gang?
Weather blasting operation is required
i)whether the blaster is authorized license holder
ii) Whether explosive used are free of defects
20 iii) Personnel are taken to respective 'SHELTER ZONE' and the area been
cordoned off and all persons maintained to a safe distance?
iv)Send information to adjoining workers/public
