The following documents are required to be submitted along with your application form. Proof of
originals is required upon submission.
 Completed application form with completed declaration by the Health Care Facility
including their official stamp
 Completed panel interview assessment report.
 Recent colored passport photograph (candidate’s picture must be true color, high quality
with high resolution )
 Passport copy (page with details , photo)
 Family Book for UAE nationals (only relevant pages including page 26)
 Resume/CV : Current/ up to date & brief
 Doctors: Valid Certificate of Good Standing (not older than 3 months)
 Visa (Residence or Visit) – valid and current
 Country Specific High school completion certificate (as per the PQR) This is not a
requirement for Physicians, Dentists and pharmacists.
 Professional qualifications – Certificate/ Diploma/ Degree/ Post Graduate
 Transcript of professional training – must include theory and clinical hours
 Practicing Professional License – valid and current from home country
 A certificate of ‘Good Standing’/ Registration or License from each country where you
have been employed to meet minimum requirement of the PQR.
 Relevant Experience to the position:
o Official certificate of service (COS) must be from HR if no HR available in the
facility it must be issued by an authorized person at health facility.
o COS must specify the position held.
o COS must specify the employment period/duration
o Scanned COS must be clear and readable.
o Date from (dd/mm/yyyy ) to (dd/mm/yyyy).
o Experience must be in a substantive post (Volunteer work and training is not
acceptable)
o Experience is calculated from the date of home country registration
 UAE MOH Evaluation/Licensure if applicable

 Once you have completed the above, you must complete the online form and submit the

For any promotion from specialist to consultant level the candidate should meet the listed
conditions:
1. Meet the PQR.
2. Performance appraisal for the last 3 years.
3. List of the hospital privileges for the last 3 years.
4. Log book for the last year as a minimum.(this should be implemented one year after the
announcement of this circular :07/01/2009 )

6. HAAD reserve the right to request the candidate to pass a panel interview if deemed
necessary by the Health Professionals Licensing Department. ( three attempts allowed with
a minimum of two months period between attempts, after failing the third attempt candidate
is not eligible for re applying till 12 months has elapsed from the date of the last interview
with evidence of further training and CME )

For candidates applying this year provided they meet all the above criteria they should be granted
the title of a consultant for one year and has to provide a full log book of cases managed before
the next renewal of license to maintain the title.

For any promotion of Medical practitioners to Specialist the candidate must meet the conditions
listed below:
1. Meet the PQR.
2. Worked in a hospital (or a medical center under supervision of a consultant) and in his
specialty field.
3. Performance appraisal for the last 3 years.
4. List of the hospital privileges for the last 3 years.
5. Recommendations of the head of department.

Renewal

Transfer

The following documents are required to be submitted along with your application form. Proof of
originals is required upon submission.

 Transfer request form completed online.

 Application singed & Stamped by old facility’s and the new facility’s Authorized
Personnel.
 Copy of the approved signature of the both parties
 Copy of the new and old Facility License.
 Resignation Letter from previous job authorized from the Facility HR.
 Appointment Letter from new Facility HR mentioning the job title
 The original license card.
 Copy of the Passport & Visa

MANDATORY REQUIREMENT FOR TRANSFER OF LOCATION:

(Either one of the below)

 Approval of both facilities on the Transfer of Location.

 Cancelation of Residence Visa.
 Cancelation of Labor Card.
 Evidence of NOT issuing Labor Card within a period of 2 months from the date of
license.
 Approval of Resignation Letter by the Authorized Signatory.
 Justice court decision in favor of the health professional to cancel his/her license.

 Once you have completed the above, you must complete the online form and submit the
hard copy of the application to our office. We will only accept and proceed with your
application once we have received all the above required documentation. We will then

Cancelation

The following documents are required to be submitted along with your application form. Proof of

 Cancelation request must be completed online within one week of the contract
termination.
 Application singed & Stamped by the Authorized Personnel.
 Copy of the Facility License.
 Copy of residence cancellation or labor card cancellation.
 Resignation Letter from previous job authorized from the Facility HR.
 The original license card.
 Copy of the Passport & Visa

 Once you have completed the above, you must complete the online form and submit the
