1. a.) Name of Organization


 Integrated Provincial Health Employees Multi-Purpose Cooperative
(IPHEMCO)

b.) Date Organized


 August 3, 1995 (CDA Registration # 9520-08009861)

c.) Why was it organized?


 To provide financial and credit services to the members of the Cooperative.

d.) What are the objectives of the organization?


 The purposes of this Cooperative is to help improve the quality of life of its
members. Towards this end, the cooperative shall aim to:
- undertake credit, savings, consumers catering, pharmacy, canteen,
lodging, laundry, janitorial services;
- generate funds and extend credit to the members for productive and
provident purposes;
- ensure the viability of the cooperatives through the utilization of new
technologies;
- encourage thrift and savings mobilization among the members;
- provide goods and services and other requirements to the members; and
- develop expertise and skills among its members.

e.) What are the current activities and projects of the organization?
 At the moment, all the scheduled activities are held in abeyance due to the
pandemic following the health directed protocol of the DOH.
f.) What are the basic duties and responsibilities of the officers and
members?
CHAIRMAN- preside over all meetings of the cooperative and of the Board
of Directors.
VICE-CHAIRMAN- in the absence or incapacity of the chairman, the Vice-
Chairman shall discharge the duties and responsibilities of the Chairman.
TREASURER- take custody of all the monies, securities and papers
acquired by the Cooperative, and maintain a complete record of all its
transactions.
SECRETARY- keep and maintain a complete registry of all members and
records/minutes of all the meetings of the Board of Directors and the General
Assembly.
MEMBERS- pay the installment of his share capital subscription as it falls
due to participate in the capital build-up and savings mobilization activities of
the Cooperative; patronize the Cooperative’s business and services; and
observe and obey all lawful orders, decisions, rules and regulations adopted by
the Board of Directors and the general assembly.

 Southern Leyte Provincial Government Employees Association
(SOLEPGEA)

b.) Date Organized


 Registered on DOLE and CSC on December 28, 2018

c.) Why was it organized?


 To promote welfare and benefits of the employees of the Provincial
Government of Southern Leyte.

d.) What are the objectives of the organization?


 The association commits itself to the pursuit of the following objectives:
- To establish an organization that will represent the employees of the
Provincial Government of Southern Leyte and its offices in collective
negotiation agreement beneficial to the members with respect to salaries,
wages and other benefits, hours of works and other terms and conditions
of employment.
- To promote the moral, social, and economic well-being of all members.
- To protect and uphold the individual and collective rights of all members
- To foster harmonious and progressive employee-administration relations.
- To enlighten the members of their rights and obligations such as their
duties and responsibilities as employee of the agency.

e.) What are the current activities and projects of the organization?
 At the moment, all the scheduled activities are held in abeyance due to the

f.) What are the basic duties and responsibilities of the officers and
members?
PRESIDENT- presides all meetings and deliberations of the Executive
Board, Board of Trustees and general membership; appoints all members of
the standing committees of the association with the concurrence of the majority
of the members of the Executive Board and Board of Trustees; approves
vouchers covering applications or expenditures of the association funds before
payments.
VICE-PRESIDENT- shall perform the duties of the President in case of
absence, undesirability, death or resignation of the President; oversee and see
to it that the operation of the association is in order and shall make a report of
the problems and recommendation to the Executive Board.
SECRETARY- records minutes of the meeting of the general membership,
Executive Board and Board of Trustees; maintain the minutes and resolution
and book of registration of all members of the association; act as custodian of
all documents, minutes of meetings, records and resolution including activities
undertaken by the association.
TREASURER- collects, receive and issue receipts for all money, funds and
contributions to the association; make sure that all the reimbursements and
payments made by him with association funds are properly documented with
supporting papers; keep records and receipts of all expenditures.
MEMBERS- are required to attend the annual General Assembly meeting,
and also scheduled special meetings; should pay the monthly dues; should
participate in cross-oriented activities held by the association like clean-up
drive, fun run, and etc.
